Foi levado ao SU do Hospital de S.Bernardo (Setúbal) após indisposição súbita após o almoço, com lipotímia e PC alguns instantes com rápida recuperação e vómito alimentar. Nega para essa altura dor abdominal ou torácica prévia ou quaisquer perdas hemáticas visíveis, embora refira dejeções de fezes escuras, tonturas, acufenos e cansaço para médios esforços há várias semanas. Há igualmente referência a sensação de distensão abdominal e obstipação de longa data.
À chegada dos bombeiros ao local, encontrar-se-ia pálido, suado e hipotenso, mas com recuperação progressiva e estado geral assintomático já na chegada do SU do HSB. A observação na admissão: Vigil, orientado, colaborante. TA 114/65mmHg; FC 60 bpm; SatO2 97% AA; Apirético. Sem alterações no ENS. AP: MV conservado sem RA. AC: Tons rítmicos mas bradicárdicos. Abd: G!oboso, mole, depressível, indolor à palpação.
Translation - English Current Clinical History
The patient was taken to the Saint Bernard emergency service in Setúbal after a sudden indisposition following lunch, presenting lipothymia and temporary LOC and vomiting. Denies at that time any previous abdominal or thoracic pain or any visible blood loss, but recalls passing dark stools and dizziness accompanied by innitus and fatigue induced by moderate effort for several weeks. There is also a reference to long term abdominal distension and obstipation.
Was found pale, sweaty and hypotense on the arrival of the fire department, but progressively recovered overall health and arrived symptom free at the emergency service of Saint Bernard Hospital. Triage assessment on admission: Alert, awake and oriented. BP 14/65mmHg; HR 60 bpm; SaO2 97% AA; Fever-free. No changes in overall physical condition. Pulmonary Auscultation (PA): Symmetrical vesicular murmurs without adventitious sounds. Arrhythmia: Regular heartbeat with bradycardia. ABD: Swollen, soft, depressible, painless upon palpation.

My family immigrated from Portugal to South Africa in 1981. I was 8. I was kept back a year in school because of the language gap, but made up for it in a month and was transferred to an advanced class, so I suppose I had a knack for it! I went on t graduate from high school with distinction and was accepted at the University of Cape Town where I enrolled in the microbiology programme. This path was sadly cut short when, in 1991, my parents decided to return to our homeland.
It seemed logical that upon my return to Portugal I should use my precious linguistic resource to further myself...so I took a degree in Accounting, logically...My career in finance made a quick ascension and I soon found myself embarking on an epic international adventure. It's been quite a journey and, while my career choices have helped me to fulfill my immediate needs, they didn't fulfill my ideals.
Needless to say, my life choices have taken me in different directions, but translation has always been with me. From translating simple texts for school colleagues to taking on larger projects with actual monetary compensation, I decided it was time I follow my heart. So, I launched myself into the unknown, abandoning the comfort of a steady income for an exciting and challenging life translating.
