Introduction: Conventional white-light endoscopy is currently the gold standard for the detection and treatment of colorectal polyps. However, up to a fifth of polyps may be missed on initial examination, especially flat and small mucosal lesions. This paper reviews the literature reporting on the use of new advances in endoscopic visualisation. Method: Literature searches were performed on Pubmed using the terms "chromoendoscopy" "narrow-band imaging" (NBI), "autofluorescence imaging" (AFI), "Fuginon Intelligent Colour Enhancement" (FICE), "i-Scan colonoscopy", "zoom colonoscopy, and " confocal laser endomicroscopy" (CLE). We focused on systematic reviews, national guidelines and randomized controlled trials written in English. Studies were assessed assessed for methodological quality using QUADAS. Prospective studies assessing new technologies were also reviewed. Further publications were identified from reference lists. Results: Chromo-endoscopy increases the detection of neoplastic polyps compared with conventional colonoscopy. (NBI) avoids the use of additional dyes and enhances the vascular network of capillaries surrounding the crypts, increasing the adenoma detection rate and the ability to distinguish between neoplastic and non-neoplastic lesions. FICE, AFI and i-Scan are new developments that improve tissue contrast. Zoom endoscopy may be combined with different modalities to help further characterise colonic lesions. CLE provides live in vivo high resolution optical sections of tissue and may be particularly useful in the surveillance of patients with long-standing ulcerative colitis, reducing the number of random biopsies. Conclusion: Although there is mounting evidence that these new technologies are superior to conventional endoscopy, current guidelines are limited. Further large scale randomised controlled trials comparing these modalities in different patient subpopulations is warranted.

Introdução: A endoscopia convencional com luz branca é, atualmente, considerada o padrão para a detecção e tratamento de pólipos colorretais. Porém, até 1/5 deles podem não ser percebidos no exame inicial, principalmente as lesões planas e pequenas. Este trabalho faz uma revisão da literatura a respeito da utilização de novos avanços em visualização endoscópica.
Método: Foram realizadas pesquisas na literatura publicada no Pubmed utilizando-se os termos “cromoendoscopia”, “narrow band imaging” (NBI) (imagem de faixa estreita), “autofluorescence imaging” (AFI) (autofluorescência) (“Fuginon Intelligent Colour Enhancement” (FICE) (Intensificação Inteligente de Cor Fuginon), “i-Scan colonoscopy” (colonoscopia ?), “zoom colonoscopy” (colonoscopia com zoom) e “confocal laser endomicroscopy” (CLE) (endomicroscopia cofocal a laser). Nosso foco recaiu sobre análises sistemáticas, diretrizes nacionais e em pesquisas controladas randomizadas escritos em língua inglesa. As pesquisas foram avaliadas em termos de qualidade metodológica utilizando QUADAS. Também foram analisados estudos prospectivos de avaliação de novas tecnologias. Outras publicações foram identificadas a partir de referências bibliográficas.
Resultados: A cromoendoscopia aumenta a detecção de pólipos neoplásicos quando comparada à colonoscopia convencional. A NBI evita a utilização de outros contrastes e realça a rede de vasos capilares em torno das criptas, elevando o índice de detecção de adenomas e a capacidade de se distinguir lesões neoplásicas das não-neoplásicas. FICE, AFI,e i-Scan são novos avanços que melhoram o contraste dos tecidos. A colonoscopia com zoom (no artigo está endoscopia) pode ser combinada com outras modalidades para ajudar na caracterização das lesões do cólon. A CLE proporciona a visualização dos tecidos in vivo, em alta resolução e pode ser particularmente útil no exame de pacientes com histórico antigo de colite ulcerativa, o que reduz o número de biópsias aleatórias.
Conclusão: Apesar de haver grande evidência de que as novas tecnologias são superiores à endoscopia convencional, as diretrizes atuais são limitadas. São necessárias mais pesquisas controladas randomizadas em larga escala dessas modalidades em subpopulações diversas de pacientes.
