Objectives To evaluate the beneficial effects of Lactobacillus reuteri (DSM 17938) in infants with functional
chronic constipation.
Study design A double-blind, placebo-controlled, randomized study was conducted from January 2008 to December 2008 in 44 consecutive infants at least 6 months old (mean age SD, 8.2 2.4 SD; male/female, 24/20) admitted to the Gastrointestinal Endoscopy and Motility Unit of the Department of Pediatrics, University ‘‘Federico II’’ of Naples, with a diagnosis of functional chronic constipation. The 44 infants with chronic constipation were randomly assigned to 2 groups: group A (n = 22) received supplementation with the probiotic L reuteri (DSM 17938) and group B (n = 22) received an identical placebo. Primary outcome measures were frequency of bowel movements per week, stool consistency, and presence of inconsolable crying episodes, recorded in a daily diary by parents.
Results Infants receiving L reuteri (DSM 17938) had a significantly higher frequency of bowel movements
than infants receiving a placebo at week 2 (P = .042), week 4 (P = .008), and week 8 (P = .027) of supplementation.
In the L reuteri group, the stool consistency was reported as hard in 19 infants (86.4%) at baseline, in 11 infants (50%) at week 2, and in 4 infants (18.2%) at weeks 4 and 8. However, there was no significant difference between L reuteri and placebo groups in the stool consistency at all weeks (P = .63, week 2; P = .38, week 4; P = .48, week 8).
Similarly, there was no statistically difference in the 2 groups in the presence of inconsolable crying episodes. No adverse effects were reported.
Conclusions The administration of L reuteri (DSM17938) in infants with chronic constipation had a positive effect on bowel frequency, even when there was no improvement in stool consistency and episodes of inconsolable crying episodes. Because of their safety profile, probiotics may be an attractive option in the treatment of functional constipation.

Source text - English Functionally, there appear to be at least two distinct families of selenium-containing enzymes. The first includes glutathione peroxidases (4) and thioredoxin reductase (5), which are involved in controlling tissue concentrations of highly reactive oxygen-containing metabolites. These metabolites are essential at low concentrations for maintaining cell-mediated immunity against infections but highly toxic if produced in excess. The role of selenium in the cytosolic enzyme glutathione peroxidase (GSHPx) was first illustrated in 1973. During stress, infection, or tissue injury, selenoenzymes may protect against the damaging effects of hydrogen peroxide or oxygen-rich free radicals. This family of enzymes catalyses the destruction of hydrogen peroxide or lipid hydroperoxides according to the following general reactions:

where GSH is glutathione and GSSG is its oxidized form. At least four forms of GSHPx exist; they differ both in their tissue distribution and in their sensitivity to selenium depletion (4). The GSHPx enzymes of liver and blood plasma fall in activity rapidly at early stages of selenium deficiency. In contrast, a form of GSHPx associated specifically with phospholipid-rich tissue membranes is preserved against selenium deficiency and is believed to have broader metabolic roles (e.g., in prostaglandin synthesis) (6). In concert with vitamin E, selenium is also involved in the protection of cell membranes against oxidative damage (see Chapter 6, Chapter 9, and Chapter 17).
The selenoenzyme thioredoxin reductase is involved in disposal of the products of oxidative metabolism (5). It contains two selenocysteine groups per molecule and is a major component of a redox system with a multiplicity of functions, among which is the capacity to degrade locally excessive and potentially toxic concentrations of peroxide and hydroperoxides likely to induce cell death and tissue atrophy (6).
Another group of selenoproteins is essential in the conversion of thyroxin, or tetraiodothyronine (T4), to its physiologically active form, triiodothyronine (T3) (7). Three types of these iodothyronine deiodinases, differing both in tissue distribution and sensitivity to selenium deficiency, have been characterised. The consequences of a low selenium status on physiologic responses to a shortage of iodine are complex. The influence of a loss of selenium-dependent iodothyronine deiodinase differs in its severity depending on whether a target tissue needs a preformed supply of T3 (e.g., via plasma) or whether, as with the brain, pituitary gland, and placenta, it can rely upon local synthesis of T3 from T4. Despite this, marked changes in the T3-T4 ratio as a consequence of a reduced selenium status (when iodine supplies are also marginal) indicate the modifying influence of selenium on thyroid hormone balance in both animal models and human subjects. Their possible significance can be anticipated from the fact that whereas thyroid weights increase typically by 50 percent in rats offered an iodine-deficient diet, thyroid weight is increased 154 percent by diets concurrently deficient in both selenium and iodine.

The outcomes will be evaluated in two ways: 1) Changes in cross-sectional values obtained for all the campers before and 3 months after camp (e.g., differences in the percent of campers with low self-efficacy as defined by the recommended score cutoff) and 2) longitudinally for each camper (e.g., change in optimism score between pre- 3- month post camp scores for each camper). For discreet variables, pre- and three-month post- camp percentages will be compared using Chi-square analyses, and for variables with small cell size, using Fisher’s exact test. Continuous variables (scales) will be compared using either Student’s t-test (for normally distributed data) or the Wilcoxon Rank Sum test for non-parametric data. If the size of the final sample permits, results will be examined for subgroups (e.g., orphans/non-orphans; boys/girls; younger/older; low/adequate compliance pre-camp, on ART/not on ART, etc.) and regression models will be tested to examine whether our measured covariates explain associations in our outcomes.
The qualitative data collected through semi-structured interviews, transcription, and translation will be examined for emergent themes using hand coding of themes based on the interview protocol. This coding will be conducted by Ms. Hiller and another SDSU graduate research assistant (TBN) who will reconcile their coding differences under supervision of the PI, add any additional appropriate codes to the coding scheme, and apply this coding scheme to two of the interview transcripts or notes to test and confirm the consistency of the qualitative coding scheme. After evaluation of all 10 dyad qualitative data records, a narrative summary will be developed describing findings, and these findings will be used to contextualize and complement quantitative questionnaire and clinical findings in the final evaluation report.
SAMPLE SIZE CALCULATION AND RECRUITMENT
Because we aim to recruit all disclosed children attending Camp Colors of Love, we hope to have almost the entire population of 80-100 children. This recruitment strategy will preclude randomization or sampling because we are working with the entire eligible population. Assuming 60% HIV antiretroviral medication adherence scores pre- intervention, which is based on data from the pilot evaluation in Ethiopia last year (n=41 children on HAART, 47 children total) we estimate that with 90 eligible participants we will be able to detect a difference as small as 10% in HIV antiretroviral adherence proportion, assuming a alpha of 0.05 and a power of 0.80.

Các kết quả sẽ được đánh giá theo hai cách: 1) Các thay đổi trên những giá trị chéo thu được từ tất cả các trại viên trước và 3 tháng sau trại (ví dụ, khác biệt về tỉ lệ phần trăm trại viên có tính hiệu quả bản thân (self-efficacy) thấp theo thang điểm khuyến nghị) và 2) đánh giá mỗi trại viên theo thời gian (ví dụ, sự thay đổi trong chỉ số lạc quan giữa thời điểm trước và 3 tháng sau trại). Các dữ liệu định tính thu thập được từ các phỏng vấn bán cấu trúc định tính, bản phiên âm và bản dịch sẽ được xem xét để tìm ra các chủ đề chung bằng cách dùng cách tự mã hoá (hand coding) các dựa theo đề cương của phỏng vấn. Việc mã hoá này sẽ được thực hiện bởi Cô Hiller một sinh viên cao học hỗ trợ nghiên cứu khác từ SDSU (TBN), người sẽ điều hoà các khác biệt trong việc mã hoá của hai bên dưới sự giám sát của PI, thêm bất kỳ mã phù hợp nào vào lược đồ mã hoá (coding scheme), và áp dụng lược đồ mã hoá này vào hai trong số các bản phiên âm hay ghi chép từ các phỏng vấn và xác định tính nhất quả của lược đồ mã hoá định tính này. Sau khi đánh giá các dữ liệu định tính của tất cả 10 cặp đôi, một bản tường thuật sẽ được thực hiện, mô tả các khám phá của nghiên cứu, và những khám phá này sẽ được ngữ cảnh hoá và bổ sung vào bảng câu hỏi định tính và các khám phá lâm sàng trong bản đánh giá sau cùng.
