Source text - Japanese 近年、スタチンによるコレステロール低下だけでなく、スタチンの血管への直接作用、いわゆるプレイオトロピックな作用が明らかにされ、コレステロール値低下という量的側面だけでなく、プラーク安定化という質的側面向上が注目されている。この観点から、血管保護作用の強いフルバスタチンのようなVascular statinと肝臓に選択的に働きコレステロール低下作用の強いスタチン(Hepatic statin)という分類が提唱され、Vascular statinの有用性が期待されている。
最近、約20,000人が参加した国内での大規模臨床試験ABCが発表された。ABC試験においては、高齢者でXXXの使用によりLDLコレステロールが非常に効率よく下がることが示された他、本来リスクが増える高齢者において非高齢者以上にイベントが抑制されたことが示され、Vascular statinの有用性が本邦での使用経験からも裏付けられた。安全性に関しても、用量の増加によっても副作用は増えず、横紋筋融解症の発現が認められないなど、XXXの高い認容性が明らかになった。注目すべき点は、LDLコレステロールが180以下であれば、イベントの増加は認められず、Vascular statinであるXXX使用では180以下に抑制されていれば、イベント抑制に十分であることが示唆されたことである。
| Translation - English Recently, not only the cholesterol lowering effect but also the direct action of statins on blood vessels, the so-called pleiotropic effect, has been made evident. Apart from reducing cholesterol, which is a quantitative aspect, attention is being paid to statins for plaque stabilization, which is a qualitative improvement. From this viewpoint, vascular statins with potent vascular protection action such as pravastatin, and statins with potent cholesterol lowering action that work selectively in the liver (hepatic statins) have been proposed. Thus, vascular statins are expected to be usefulness.
Recently, a large-scale Japanese clinical study called ABC in about 20,000 participating subjects was announced. The ABC study demonstrated that use of XXX in the elderly very effectively lowered LDL cholesterol and that adverse events in the non-elderly and elderly, who are at an increasing risk, was suppressed. Usefulness of vascular statins was also supported by the clinical experience in Japan. With regard to safety, an increase in dose is not accompanied with an increase in adverse reactions and there has been no onset of rhabdomyolysis. Thus, the high tolerance of XXX became evident. Of note, LDL cholesterol of ≤180 was not accompanied by an increase in adverse event, demonstrating that use of XXX, which is a vascular statin, produced adequate inhibition of events when LDL cholesterol was reduced to ≤180.
