Source text - English Methadone is a strong mu opioid receptor (MOR) agonist. It antagonizes glutamate by blocking the N-methyl-D-aspartate receptor (NMDA) and inhibits the reuptake of serotonin and noradrenaline in the central nervous system. When used parenterally, its latency is of 10 to 20 minutes (min), with 4 to 36 hours (h) duration of action, depending on the dosage, due to its long half-life and the large individual variation10,11. When administered in doses of 20 to 30 mg, analgesia generated by intravenous (i.v.) methadone can last from 24 to 36 h12.
Due to these characteristics, methadone may be an interesting option in the treatment of postoperative pain, and the dose used in anesthesia may contribute to immediate postoperative analgesia, when the highest pain scores are usually reported. Few published studies have compared intraoperative methadone to fentanyl or sufentanil in cardiac surgery13,14, spine surgery15, laparoscopy16, hysterectomy17,18 abdominal surgery18,19 and major surgery in children20. To our knowledge, no study has compared the use of intraoperative methadone with other opioids for the morbidly obese patient.
The present study was designed to evaluate the efficacy and safety of intraoperative i.v. methadone in reducing postoperative pain and opioid consumption in morbidly obese patients submitted to open bariatric surgery.

Quando o uso é parenteral, sua latência é de 10 a 20 minutos (min), com duração de ação de 4 a 36 horas (h), dependendo da dose, devido à sua longa meia-vida e à grande variação individual10,11. Quando administrada em doses de 20 a 30 mg, sua analgesia intravenosa (i.v.) pode durar de 24 a 36 h12.
Devido a essas características, a metadona pode ser uma opção interessante no tratamento da dor pós-operatória, e a dose usada em anestesia pode contribuir para a analgesia pós-operatória imediata, quando os maiores níveis de dor são geralmente referidos. Poucos estudos publicados compararam o uso intraoperatório de metadona com fentanil ou sufentanil em cirurgia cardíaca13,14, cirurgia de coluna15, laparoscopia16, histerectomia17,18, cirurgia abdominal18,19 e cirurgia pediátrica de grande porte20. De acordo com nosso conhecimento, nenhum estudo comparou o uso intraoperatório de metadona com o de outros opióides em pacientes obesos mórbidos.
O presente estudo teve como objetivo avaliar a eficácia e segurança do uso i.v. intraoperatório de metadona na redução da dor pós-operatória e do consumo de opióide em pacientes obesos mórbidos submetidos a cirurgia bariátrica aberta.
