Source text - Turkish Melatonin uygulanan ratların incebağırsaklarında (duodenum, jejunum, ileum) motilin ve ghrelinin immünohistokimyasal lokalizasyonu.
Özet
Çalışmamızın amacı melatoninin incebağırsak dokusu üzerine etkilerini immünohistokimyasal, histolojik ve histometrik yöntemlerle incelemekti. Bu amaçla 10- 12 haftalık 30 adet Spraque Dawley ratlar kullanıldı. Kontrol grubu, Sham grubu ve Deneme (Melatonin) grubu olmak üzere, rastgele ve sistematik yöntemle, üç grup oluşturuldu. Denemeye ait ratlara etanolde çözdürülüp serum fizyolojikle sulandırılmış olan 10 mg/kg dozdaki melatonin 21 gün süre ile intraperitoneal yolla günlük olarak enjekte edildi. Sham grubuna 21 gün süre ile her gün deneme grubuna uygulanan miktarda etanol ve serum fizyolojik intraperitoneal yol ile uygulandı. Kontrol grubuna ise hiçbir uygulama yapılmadı. Deneysel sürecin sonunda alınan incebağırsak dokuları üzerinde histometrik, immünohistokimyasal ve semikantitatif incelemeler yapıldı. Sonuç olarak; histometrik incelemelerde; duodenumda villus uzunluğu ve eni için melatonin grubu, kontrol ve sham grubu ile karşılaştırıldığında anlamlı fark bulundu. Jejunumda ise sadece villus uzunluğu bakımından melatonin grubu ile diğer gruplar arasında anlamlı fark bulundu. İleumda ise gruplar arasında anlamlı bir fark bulunmadı. İmmünohistokimyasal olarak melatonin uygulanan grupta duodenum, jejunum ve ileumda, motilin ve ghrelin immünoreaktivitesinin azaldığı tespit edildi.

Abstract
The aim of our study was to examine the effects of melatonin on the small intestine tissue with immunohistochemical, histological and histometric methods. For this purpose, 30 pieces of Spraque Dawley rats, which were 10-12 weeks old, were used. Three groups were formed randomly and systematically, namely the control group, the Sham group and the Trial (Melatonin) group. The rats belonging to the experiment were injected daily with the melatonin at a dose of 10 mg/kg, dissolved in ethanol and diluted with the physiological saline, with the intraperitoneal way for 21 days. Ethanol and physiological saline were administered to the sham group with the intraperitoneal way every day for 21 days in the amount administered to the experimental group. No administration was made to the control group. The histometric, immunohistochemical and semi-quantitative analyzes were performed on the small intestine tissues taken at the end of the experimental process. As a result, a significant difference was found in the duodenum villus length and width when the melatonin group was compared with the control and sham groups in the histometric examinations. In the jejunum, a significant difference was found between the melatonin group and the other groups only in terms of villus length. There was no significant difference between the groups in the ileum. It was determined that the motilin and ghrelin immunoreactivity were decreased in the duodenum, jejunum and ileum in the group administered melatonin immunohistochemically.

Source text - Turkish Osteomiyelit, klinisyenler için sıklıkla tanısal zorluk oluşturan, giderek yaygınlaşan bir hastalıktır. Yaşam veya uzuv kaybına neden olabilecek komplikasyonları önlemek için doğru ve zamanında tanı çok önemlidir. Hikâye, fiziksel muayene ve laboratuvar çalışmalarının yanı sıra, tanısal görüntüleme de tanı sürecinde çok önemlidir. Bu anlatı inceleme makalesinde, osteomiyelit tanısı koymak için kullanılan çeşitli görüntüleme yöntemleri, yani düz radyografi, bilgisayarlı tomografi (BT), manyetik rezonans görüntüleme (MRG), ultrasonografi, kemik sintigrafisi ve pozitron emisyon tomografisi (PET) ele alınmaktadır. PubMed'den makaleler elde edilmiş ve "osteomiyelit için tanısal görüntüleme” konusuyla ilgisi açısından taranmıştır. Yazarlar, görüntüler osteolitik değişiklikleri ortaya çıkarabileceğinden ve alternatif hastalıkları dışlamaya yardımcı olabileceğinden, düz radyografinin uygun bir ilk adım olduğu sonucuna varmıştır. Hassas olması ve kemik iliği değişikliklerini enfeksiyondan sonraki birkaç gün içinde ortaya çıkarabilmesi nedeniyle en uygun ikinci adım genellikle MRG’dir. MRG yapılamayan hastalarda BT, ultrasonografi ve kemik sintigrafisi gibi diğer görüntüleme yöntemleri faydalı olabilir. BT, kronik enfeksiyonlarda nekrotik kemiği tanımlamak için faydalıdır. Ultrasonografi, orak hücre hastalığı olan çocuklarda veya kişilerde faydalı olabilir. Kemik sintigrafisi özellikle vertebral osteomiyelitin saptanmasında faydalıdır. Son olarak, PET yüksek hassasiyet ve özgüllük sergiler, ancak klinik uygulaması, yüksek maliyeti ve yetersiz düzeyde bulunabilir olması nedeniyle sınırlıdır. Uygun kullanıldığında tanısal görüntüleme, osteomiyeliti saptamak için yüksek düzeyde hassas ve spesifik bir yöntem olabilir, bu da onu güçten düşüren bu hastalığın tanı sürecinde çok önemli bir adım haline getirir.
Translation - English Osteomyelitis is an increasingly common condition that possesses a diagnostic challenge to the clinicians. Accurate and timely diagnosis is crucial for preventing complications that can result in the loss of life or limb. In addition to history, physical examination, and laboratory studies, diagnostic imaging is essential in the diagnostic process. In this narrative review article, various imaging modalities to diagnose osteomyelitis are described; these include plain radiography, computed tomography (CT), magnetic resonance imaging (MRI), ultrasonography, bone scintigraphy, and positron emission tomography (PET). Articles were identified through PubMed and screened for relevance to the topic of diagnostic imaging for osteomyelitis. The authors concluded that plain radiography is an appropriate first step because the images may reveal osteolytic changes and can help rule out alternative disease. MRI is often the most appropriate second step because it is highly sensitive and can reveal bone marrow changes within days of an infection. Other imaging modalities such as CT, ultrasonography, and bone scintigraphy may be useful for patients in who MRI cannot be performed. CT is useful for identifying necrotic bone in chronic infections. Ultrasonography may be useful in children or individuals with sickle-cell disease. Bone scintigraphy is particularly useful in detecting vertebral osteomyelitis. Finally, PET has demonstrated high sensitivity and specificity; however, as it is expensive and often unavailable its clinical application is limited. When used appropriately, radiographic evaluation has high sensitivity and specificity for detecting osteomyelitis; thus, it is a crucial step in diagnosing this debilitating condition.

Source text - Turkish Leiomyosarkom (LMS), muskularis mukozasından veya muskularis propriadan kaynaklanan, nadir bir ince bağırsak tümörüdür. İnce bağırsakta LMS’nin en sık görüldüğü bölge jejunum olup, bunu ileum ve duodenum takip eder. Yaygın belirtileri arasında karında kitle, karın ağrısı ve açık gastrointestinal kanama bulunur. LMS, çoğunlukla yaşamın 6. on yılında ortaya çıkar ve erkeklerde bir miktar daha baskındır. İnce bağırsak tümörlerinin preoperatif tanısı, özellikle benign ve malign tümörleri ayırt etme açısından zordur. Yakın tarihli bir literatür taraması, bilgisayarlı tomografi (BT) ve manyetik rezonans (MR) enterografi ve enteroklizinin, ince bağırsakta LMS'nin değerlendirilmesi için iyi yöntemler olduğunu ortaya koymuştur. Hem BT hem de MR görüntülemede atlanabilen yüzeysel lezyonlar, kapsül endoskopi ile yaklaşık %80’lik bir saptama oranıyla saptanabilir. Translation - English Leiomyosarcoma (LMS) is a rare small intestinal tumor, which arises from the muscularis mucosa or muscularis propria . The most common site of occurrence of LMS in the small intestine is the jejunum, followed by the ileum and duodenum. Its common manifestations include abdominal mass, abdominal pain, and overt gastrointestinal bleeding.. They are mainly observed in the 6th decade of life, with slight male preponderance. In general, the preoperative diagnosis of small intestinal tumors such as LMSs is difficult, especially in terms of differentiating between benign and malignant tumors. According to a recent literature review computed tomography (CT) and magnetic resonance (MR) enterography and enteroclysis are good modalities for the assessment of LMS. Superficial lesions, which can be missed by both CT and MR imaging, can be detected by water capsule endoscopy, with a detection rate of approximately 80%. Histologically, LMS resembles gastrointestinal stromal tumor; however, on immunohistochemical analysis, it has been found to be negative for CD117 and CD34 and positive for smooth muscle actin and desmin. When the size of LMS is more than 5 cm, it can hematogenously spread to the liver (65%), other gastrointestinal organs (15%), and the lungs (4%). It can also spread via the lymphatic system (13%) or peritoneal route (18%). The response of LMS to chemotherapy is unknown, and radiotherapy does not play a role in the treatment. Therefore, surgery is the only effective treatment for LMS in the small intestine. The primary tumor should be excised radically with wide resection of the mesentery. If possible, metastasectomy should be considered. Large phase II and III trials involving the combination of docetaxel and gemcitabine have reported impressive response rates for LMSs (mostly of uterine origin). However, some studies have not been able to confirm the efficacy of this combination. Trabectedin has recently showed response rates of up to 56% for LMSs, and it appeared to be particularly useful against far-advanced and metastatic LMSs after failure of anthracyclines and ifosfamide combination therapy.
