El síndrome del túnel carpiano (STC) es una neuropatía producida por el atrapamiento y compresión del nervio mediano en la muñeca a su paso por un túnel fibroóseo constituido por el ligamento transverso y los huesos del carpo. Habitualmente cursa con parestesias y dolor en el territorio del nervio mediano, siendo la debilidad, hipoestesia y atrofia tenar signos mas tardíos; el diagnóstico se confirma por los hallazgos de la exploración física y del electromiograma. La etiología es muy diversa, siendo las causas mas frecuentes: ocupacional, postraumatismos, diabetes mellitus, artritis reumatoide, idiopática, amiloidosis y diversos trastornos endocrinos. La tuberculosis (TB) se encuentra entre la larga lista de trastornos que con escasa frecuencia pueden dar lugar al síndrome del túnel carpiano, y puede ser excepcionalmente la primera manifestación de la enfermedad o incluso bilateral. Recientemente tuvimos la oportunidad de observar un paciente con síndrome del túnel carpiano, en quien este constituyó la primera y única manifestación de una TB, y dada la rareza de esta circunstancia nos ha parecido de interés la comunicación del caso.
Paciente de 52 años de edad, sin antecedentes de especial interés, que consulta por parestesias y debilidad de los dedos pulgar e índice de la mano derecha desde 5 o 6 años antes, y dolor y tumefacci6n en la muñeca derecha en los últimos dos años, junto con hipoestesia en los dedos pulgar e índice del mismo lado. En la exploración física destacaba una marcada disminución de la sensibilidad en el territorio del nervio mediano derecho con signa de Tinel positivo, atrofia tenar y moderada pérdida de fuerza en la aducción del pulgar derecho. El resto de la exploración era anodino.

Carpal tunnel syndrome (CTS) is a neuropathy produced by the entrapment and compression of the median nerve in the wrist when it passes through a fibro-osseous tunnel constituted by the transverse ligament and the carpal bones. It is usually accompanied by paresthesia and pain in the median nerve region, with later signs being debility, hypoesthesia and thenar atrophy; the diagnosis is confirmed by findings from physical examination and from the electromyogram. The etiology is very diverse, most common causes being: occupational, post-trauma, diabetes mellitus, rheumatoid arthritis, idiopathic, amyloidosis, and diverse endocrine disorders. Tuberculosis (TB) is amongst the long list of disorders that infrequency can lead to carpal tunnel syndrome, and exceptionally can be the first or even bilateral manifestation of the illness. Recently we had the opportunity to observe a patient with carpal tunnel syndrome who represented the first and only manifestation of a TB, and given the rarity of this circumstance, communicating the case has been of our interest.
Fifty two year old patient, with no previous case of special interest, who consults for paresthesia and weakness in the thumb and index fingers of the right hand for the last 5 or 6 years, and pain and tumefaction in the right wrist in the last two years, combined with hypoesthesia in the thumb and index fingers on the same side. In the physical examination, a marked decrease in feeling in the right median nerve region was noted with a positive Tinel’s sign, thenar atrophy, and a moderate loss of strength in right thumb adduction. The rest of the examination was anodyne.

It is well appreciated that individuals often wish to cosmetically alter the coloring of their hair by various known hair coloring treatments and that many individuals change their own hair color to their favorite hair color using currently available hair dye compositions. In particular, oxidative dye compositions are commonly used because such compositions simultaneously decolor and dye hair, thus widening the degree of freedom of hair color. Thus, individuals having comparatively dark hair are able to use such products. Moreover, the products provide a greater number of color variations.
Use of oxidative dye compositions to treat hair is typically not inexpensive and is time consuming as well. Moreover, the chemicals employed in these treatments can be caustic and somewhat damaging to the hair. Their frequent use is not preferred, and rather they are generally used at intervals of at least about one month. It is unavoidable that the color of hair dyed with oxidative dye compositions gradually fades during this one month owing to the influence of washing with shampoo, sweat, ultraviolet rays from sunshine and the like. The gradual fading of color results in a gradual reduction in the feeling of satisfaction generally felt immediately after dyeing.
