The use of single-dose NVP to prevent perinatal HIV transmission has been shown to be very effective, but is associated with the selection of NNRTI-resistant virus. To prevent selection of resistant virus while efficiently preventing perinatal transmission, alternative approaches can also be considered and should be compared with the P1032 approach.
• Replacing NVP with a drug as potent and rapid in action as NVP but with a high resistance barrier would prevent the development of resistance mutations. However, to date, the dramatic reduction of perinatal HIV transmission rates obtained with the ZDV/NVP combination has not been observed with any other simple regimens. In particular, the combination of NVP and ZDV/3TC has been associated with a high incidence of 3TC resistance mutations. Tenofovir may be shown to meet this need in the future.
• Initiating highly active antiretroviral therapy (HAART) in women who are identified as HIV-infected during pregnancy and have clinical indications for ARV treatment combinations for their own disease, as recommended by the World Health Organization (WHO) 15. This strategy, which has the advantage of avoiding the use of single-dose NVP, should be as efficacious as ZDV plus NVP for the prevention of perinatal HIV transmission. However, 75% of pregnant women do not urgently need HAART for their own health because they are not immunocompromised. It is unclear whether these women would benefit more from HAART given only during pregnancy than from highly effective, safe, and simpler regimens that are designed specifically for the prevention of perinatal HIV transmission. Moreover, initiating HAART during pregnancy, even in immunocompromised women, is not always feasible for logistical reasons and because some women do not seek antenatal care. In addition, unique safety issues for both the mother and the child still exist and are complicated by the high prevalence of chronic Hepatitis B in Asia, which makes the initiation of NVP-based HAART during pregnancy unsafe. Therefore, initiating HAART during pregnancy should probably be encouraged for immunocompromised women who will continue HAART after delivery, but this solution is not appropriate for all women.
• Delaying the initiation of NNRTI-based HAART as long as possible after delivery might be another strategy. Resistance mutations observed shortly after single-dose NVP exposure tend to fade over time 7. Thus, delaying the initiation of HAART as long as possible may increase the chances of success of subsequent NNRTI-based therapies. However, this approach still needs to be assessed.
If delaying the initiation of HAART is not possible for more than a few months, using a first line regimen that does not include any NNRTI may also increase the chances of success of a NNRTI based regimen later on.
In conclusion, none of these alternative approaches can provide a universal solution to the problem of acquisition of NVP resistance mutations in the context of PMTCT. P1032 will provide specific answers to a problem that cannot be always avoided. Current HIV treatment guidelines in Thailand recommend initiating treatment for immunocompromised individuals. The primary drug combination used is d4T/3TC/NVP, which is formulated as a single pill. The widespread use of this combination highlights the importance of avoiding the selection of NVP-resistant virus, which could predispose to treatment failure when treatment with this combination is subsequently initiated.
Preliminary data from the Treatment Options Preservation Study (TOPS), an ongoing study of postpartum ZDV/3TC for preventing the emergence of NVP resistance mutations following single-dose intrapartum NVP, were presented at the XV International AIDS Conference in July 2004. In this study, treatment-naïve women in South Africa were randomized to receive one of three regimens: single-dose NVP alone (Arm 1), single-dose NVP plus four days of postpartum ZDV/3TC (Arm 2), or single-dose NVP plus seven days of postpartum ZDV/3TC (Arm 3). Data on 61 women were presented. At six weeks postpartum, NNRTI resistance mutations were identified by consensus sequencing in 8/15 (53.3%) women in Arm 1, 1/20 (5.0%) women in Arm 2, and 3/22 (13.6%) women in Arm 3. The difference in the rate of resistance between Arm 1 and Arms 2 and 3 combined was significant in a preliminary analysis. In this population infected with clade C virus, the most common NNRTI mutations were K103N, Y181C, and A190G. By sequencing of single viral templates, by diluting the specimen to end-point, higher rates of resistance were found in all arms 16. Based on these preliminary results, the TOPS study closed Arm 1.
Further data on the TOPS study with a larger number of subjects were presented at the 3rd International AIDS Society Conference on HIV Pathogenesis and Treatment in July 2005. Two hundred twenty-six women with 6 week follow-up were evaluated. The median entry CD4 count was 314 cells/mm3; median viral load was 4.49 log10 copies/mL. Population sequencing of 2 and 6 week specimens showed NNRTI resistance in 39/68 (57%) women in Arm 1; 9/67 (13%) and 6/68 (9%) in Arms 2 and 3, respectively. The most common NNRTI mutations were K103N, Y188C, Y181C, V106M, A190G, and V106A. No NRTI mutations, including 184, were detected17.
Following the results of the TOPS study, the P1032 Protocol Team believes that it is not feasible to include a NVP-alone control arm in P1032, since the addition of additional ART has been shown to be effective in preventing the emergence of NVP resistance mutations. However, we recognize that there will be differences between the women enrolled in TOPS and P1032. These differences will include the frequent use of prenatal ZDV in the P1032 population, different HIV subtypes, and likely differences in HIV RNA viral load and CD4 count as well. Thus, we elected to use a historical control group (women enrolled in the Thai PHPT-2 trial) to determine the rate of NVP resistance mutations following single-dose NVP. The PHPT-2 study is a randomized trial of single-dose NVP with antenatal ZDV conducted in 2001-2002 at study sites where P1032 will be implemented, so the women enrolled in the PHPT-2 study will be similar to the women enrolled in P1032.

Source text - English GENERAL DESCRIPTION AND SYSTEM OPERATION
IGNITION SYSTEM OPERATION
This ignition system does not use a conventional distributor and coil. It uses a crankshaft position sensor input to the electronic control module (ECM). The ECM then determines electronic spark timing (EST) and triggers the direct ignition system ignition coil.
This type of distributorless ignition system uses a ``waste spark'' method of spark distribution. Each cylinder is paired with the cylinder that is opposite it (1-4 or 2-3). The spark occurs simultaneously in the cylinder coming up on the compression stroke and in the cylinder coming up on the exhaust stroke. The cylinder on the exhaust stroke requires very little of the available energy to fire the spark plug. The remaining energy is available to the spark plug in the cylinder on the compression stroke.
These systems use the EST signal from the ECM to control the electronic spark timing. The ECM uses the following information:
• Engine load (manifold pressure or vacuum).
• Atmospheric (barometric) pressure.
• Engine temperature.
• Intake air temperature.
• Crankshaft position.
• Engine speed (rpm).
DIRECT IGNITION SYSTEM IGNITION COIL
The direct ignition system (DIS) ignition coil is mounted near the rear of the camshaft carrier on the single overhead camshaft engine. On the dual overhead camshaft engine, the DIS ignition coil is mounted near the rear
of the cylinder head. Each pair of terminals of the DIS ignition coil provides the spark for two spark plugs simultaneously. The DIS ignition coil is not serviceable and must be replaced as an assembly.
CRANKSHAFT POSITION SENSOR
This direct ignition system uses a magnetic crankshaft position sensor. This sensor protrudes through its mount to within approximately 1.3 mm (0.05 inch) of the crankshaft reluctor. The reluctor is a special wheel attached to the crankshaft or crankshaft pulley with 58 slots machined into it, 57 of which are equally spaced in 6 degree intervals. The last slot is wider and serves to generate a "sync pulse." As the crankshaft rotates, the slots in the reluctor change the magnetic field of the sensor, creating an induced voltage pulse. The longer pulse of the 58th slot identifies a specific orientation of the crankshaft and allows the electronic control module (ECM) to determine the crankshaft orientation at all times. The ECM uses this information to generate timed ignition and injection pulses that it sends to the ignition coils and to the fuel injectors.
IDLE AIR SYSTEM OPERATION
The idle air system operation is controlled by the base idle setting of the throttle body and the idle air control (IAC) valve.
The electronic control module (ECM) uses the IAC valve to set the idle speed dependent on conditions. The ECM uses information from various inputs, such as coolant temperature, manifold vacuum, etc., for the effective control of the idle speed.
FUEL CONTROL SYSTEM OPERATION
The function of the fuel metering system is to deliver the correct amount of fuel to the engine under all operating conditions. The fuel is delivered to the engine by the individual fuel injectors mounted into the intake manifold near each cylinder.
The two main fuel control sensors are the manifold absolute pressure (MAP) sensor and the oxygen (O2) sensor.
The MAP sensor measures or senses the intake manifold vacuum. Under high fuel demands the MAP sensor reads a low vacuum condition, such as wide open throttle. The electronic control module (ECM) uses this information to richen the mixture, thus increasing the fuel injector on-time, to provide the correct amount of fuel. When decelerating, the vacuum increases. This vacuum change is sensed by the MAP sensor and read by the ECM, which then decreases the fuel injector on-time due to the low fuel demand conditions.
The O2 sensor is located in the exhaust manifold. The O2 sensor indicates to the ECM the amount of oxygen in the exhaust gas and the ECM changes the air/fuel ratio to the engine by controlling the fuel injectors. The best air/fuel ratio to minimize exhaust emissions is 14.7 to 1, which allows the catalytic converter to operate most efficiently. Because of the constant measuring and adjusting of the air/fuel ratio, the fuel injection system is called a "closed loop" system.
