Homicide chez les malades mentaux en Tunisie : Aspects épidémio-cliniques
Résumé
Introduction : La dangerosité criminelle est souvent associée à des troubles mentaux. Cette association semble d’autant plus forte en présence de facteurs de risque de comportement violent chez un malade mental.
Le but de ce travail est de tenter, d’une part de dégager les données épidémiologiques générales concernant la relation entre homicide et pathologie psychiatrique et d’autre part d’étudier ces données en fonction de leurs aspects cliniques et de les aborder sur le plan criminologique.
Méthodes : une étude épidémiologique, rétrospective et descriptive , elle a porté sur 50 meurtriers malades mentaux hospitalisés d’office entre Février 2002 et Novembre 2012 dans l’hôpital psychiatrique de Tunis Razi à la suite d’un non-lieu pour «cause de démence ». On a eu recours pour cela aux dossiers médicaux, aux expertises psychiatriques pénales, aux éventuels rapports d’autopsie médicolégale et aux rapports de fin d’instruction judiciaires.
Résultats : Notre population se composait de 42 hommes et 8 femmes, l’âge moyen était de 36,3 ± 10,8 ans. Le profil des meurtriers de notre série se caractérise par un statut sociodémographique particulier : célibataires (64%), sans profession (58%), niveau d’instruction primaire (50%), niveau socioéconomique défavorable (58%) et par des antécédents psychiatriques chargés (66%) dont 72 ,7% étaient des schizophrènes, avec une mauvaise observance thérapeutique (82%). Avant l’acte commis d’homicide 22% avaient déjà un antécédent judiciaire, et ceci pour des raisons de violence dans 72,7% des cas. On a enregistré au sein de notre population : 62% de schizophrènes, 26% étaient atteints de trouble délirant, 6% de trouble dépressif et 6% avaient un retard mental. La victime était connue de l'auteur dans 80% des cas. Nous avons recensé 30% de parricides.
Nous avons retrouvé que 58% de nos patients consommaient de l’alcool et 18% présentaient un usage de stupéfiant, d’abus et de dépendances confondus.
Conclusion : Le risque de passage à l’acte d’homicide chez les malades mentaux tient au tant à la pathologie psychiatrique qu’à d’autres facteurs tels que la non observance thérapeutique, la rupture de soin, la marginalisation, la comorbidité d’alcool ou d’autres substances et les troubles de la personnalité.

Abstract
Background:
Criminal dangerousness is often associated with mental disorders. This association seems even stronger in presence of violent behavior risk factors among mentally ill patients.
The aim of this work is to try on the one hand to identify the general epidemiological data regarding the relationship between homicide and psychiatric pathology, and on the other to study these data according to their clinical aspects and address them in criminological terms.
Methods :
It is a retrospective and descriptive epidemiological study. It involved 50 mentally ill murderers who were automatically hospitalized between February 2002 and November 2012 in Razi psychiatric hospital of Tunis after the cases were dismissed "because of dementia ".
We used medical records, criminal psychiatric assessments, potential medicolegal autopsy reports and end of judicial instruction reports.
Results:
Our population consisted of 42 men and 8 women, mean age 36.3 ± 10.8 years. The profile of murderers in our series is characterized by a particular sociodemographic status: single (64%), unemployed (58%), primary education level (50%), low socioeconomic level (58%) and a loaded psychiatric history (66%) in which (72, 7%) were schizophrenic, with poor adherence to therapy (82%). Before the act of homicide (22%) had a criminal record because of violence in (72.7%) of cases. We have recorded in our population: (62%) of schizophrenic patients, (26%) were suffering from delusional disorder, depressive disorder (6%) and (6%) had intellectual disability. The victims were known by the perpetrator in (80%) of cases. We identified (30%) of parricides. We found that (58%) of our patients drank alcohol and (18%) used drugs, they presented different kinds of abuse and addiction.
Conclusion :
The risk of committing a homicide among the mentally ill is related to both psychiatric illness and other factors such as non-adherence to therapy, a rupture in treatment, marginalization, comorbidity to alcohol or other substances and personality disorders.

Introduction :
Le vieillissement de la population est un phénomène caractéristique des pays occidentaux et bientôt la Tunisie. Une moyenne de 20 % de la population âgée de plus de 60 ans est prévue pour 2029.
La dépression chez les personnes âgées est fréquente, en particulier chez ceux qui vivent en institutions.
Objectifs :
• Déterminer la prévalence de la dépression chez les sujets âgés vivant en institution.
• Evaluer l’impact des facteurs sociodémographiques sur la prévalence de la dépression chez le sujet âgé en institution.
Patients et méthodes :
Etude transversale descriptive au centre de résidence des sujets âgés de la Manouba.
Dépistage de la dépression par l’échelle Geriatrique Depression Scale (GDS 15).
L’indépendance dans les actes de la vie quotidienne a été évaluée par l’échelle Activities of Daily Living (ADL)
Recueil des caractéristiques Sociodémographiques par un questionnaire préétabli.
Résultats :
• Quarante deux sujets ont été inclus dans l’étude.
• Les participants avaient en moyenne 71,5 ans.
• Soixante-treize pourcents des participants étaient de sexe masculin.
• Dix-sept pourcent présentaient une dépression, 45,2% présentaient une dépression probable contre 37,8 qui ne présentaient pas de dépression.
• Soixante six virgule deux pourcent étaient autonomes.
• Les facteurs significativement associés à la dépression étaient : le sexe féminin (p=0,048), l’origine citadine (p=0,05).
• Le degré d’autonomie n’est pas un facteur influençant la dépression du sujet âgé en institution.
Conclusion : Le dépistage de la dépression chez les sujets âgés en institution s’impose en particulier chez les sujets à risque.

Conclusion:
Screening for depression in the institutionalized elderly is particularly essential in patients at risk.

Source text - English Special Olympics athlete Yao Armand and the national Unified Football Team from Côte d'Ivoire had a successful campaign in South Africa in October by finishing third with a bronze medal and qualifying for a worldwide football competition to be held next year in Brazil.
But that was just one of the highlights Yao will always remember from his trip to South Africa. He also received a prescription for new glasses through the Special Olympics Opening Eyes program, something that volunteer eyes doctors expect will dramatically improve his eyesight, his future performances on the field, and virtually every other aspect of his life.
“This is a population that has gone without for far too long,” Her Excellency Joyce Banda, president of the African nation of Malawi, said in a recent address. “It is our obligation to expose injustice and right what is wrong. An inclusionary approach is essential to sustainable development, which must be development for all.”
In Malawi, words like these are more than just window dressing; they’re policy. In her first 100 days in office, Banda passed the Malawi Disability Act, and the country has created a ministry dedicated to people with disabilities and aligned itself to comply with the UN Convention on the Rights of Persons with Disabilities.
Banda also committed to forming a national partnership with Special Olympics Malawi. The country has one of the most innovative national Programs in the Special Olympics movement, supporting more than 5000 active athletes, despite being relatively small and rural.
Last year, Kansas was named a Special Olympics Healthy Community for its innovative ways of weaving health into its programming and ensuring that athlete wellbeing is top of mind. One initiative that proves this is the case is Bike4Life.
Athletes in the program have become more confident, happier, and healthier by working to lose weight.
“Seeing where these athletes are now, compared to just a year ago – there’s a huge difference.” said John Lair, a Special Olympics coach. “We have parents and volunteers saying all the time how much of a difference this program has made.”
Support from the Kansas Health Foundation has helped make Bike4Life possible, and Special Olympics Kansas is working to find new partners, so even more athletes can make health a major focus in their lives.

du had expanded its digital presence and moved its customer portal, www. du.com onto Progress Sitefinity™ CMS more than four years ago. At the time, Sitefinity CMS was chosen for its ease of use and robust features. However, with mobile on the rise, du decided to rebuild its website from scratch.
Challenge

I am a native Arabic linguist from Tunisia. I am fluent in French and English. I hold a master's degree in linguistics from the university of human and social sciences of Tunis.
My specialism is IT as I have been translating with success complicated software related documents from English to Arabic. Moreover, I work on business texts such press releases and marketing materials and legal documents mainly birth certificates, passport, driving licence and marriage certificate . I also translate general medical documents.
Before accepting any project I evaluate the time and energy needed to bring it to fruition, as a result, I always respect the deadlines of the work I claim.
I am not only delivery focused but detail-oriented, and responsive. I always keep improving my internet search skills and look for new reference sites and books for general translations and niche projects.
I have successfully translated more than three hundred thousand words used SDL Studio. Besides, I use Transcribe! for transcription, Aegisub for subtitling and Audacity for voice-over.
