Source text - English The development of percutaneous coronary intervention (PCI) with stet implantation has revolutionized the practice of cardiology over the course of the past decades. However, despite considerable technological advancements, in-stent restenosis (ISR) remains the most common cause of treatment failure after PCI. Moreover the high efficacy with contemporary devices – primarily drug-eluting stents (DES) – has facilitated the expansion of PCI to broader and increasingly more complex lesion and patient subsets. Accordingly, despite low rates of ISR in relative terms the absolute numbers of patients presenting with stent failure remain considerable. Importantly the management of this condition remains challenging, with high rates of subsequent events at medium- to long-term follow-up.
The term restenosis is used in a variety of settings across the field of interventional cardiology. Angiographic restenosis is commonly adjudicated as a binary event defined as a re-narrowing of more than 50% of the vessel diameter as determined by coronary angiography. As this definition is based on two-dimensional parameters accurate measurements are critically dependent on the acquisition of worst-view projections. Typically visual estimation of restenosis is employed in routine clinical practice in the catheterization laboratory. This requires the operator to develop a sense of what comprises a 50% diameter stenosis. In adjudication of ISR the basic frame of reference is the body of the stent – this is known as an in-stent analysis. However, restenosis also shows a predilection for occurrence at stent margins. Accordingly a frame of reference including both the body of the stent and 5-mm margin proximal and distal to the stent edges is also usually assessed – this is known as an in-segment analysis.

The cerebellum i s composed of two hemispheres joined in the midline by the vermis (Fig. 17.1 ). The cerebellum has three surfaces: suboccipital, tentorial, and petrosal. Each of the surfaces is divided by a major fissure named according to its surface: suboccipital, tentorial, and petrosal fissures. The hemispheric part of the tentorial surface in composed of the quadrangular, simple, and superior semilunar lobules. The tentorial surface is divided into anterior and posterior parts by its major fissure, the tentorial (primary) fissure. This fissure separates the quadrangular and simple lobules on the hemisphere, and the culmen and declive on the vermis. The postclival fissure separates the simple and superior semilunar lobules.
The hemispheric portion of the suboccipital surface is composed of the superior semilunar lobule, inferior semilunar lobule, biventral lobule, and tonsil. The suboccipital surface is divided into superior and inferior parts by its major fissure, the suboccipital fissure. The vermian part of this fissure is called the prepyramidal fissure, and the hemispheric part of this fissure is called the prebiventral fissure. Minor fissures on the suboccipital surface include the petrosal (horizontal) fissure between the superior and inferior semilunar lobules, which is an extension of the major fissure on the petrosal surface, and the tonsillobiventral fissure.

Risk factors/Pathogenesis
- Sunlight: UVB strongly correlated w/SCC; correlation w/BCC is more compelx
- Fair-skinned individuals at greatest risk
- Radiation exposure at a young age
- Prolonged PUVA exposure
- Immunosuppression (organ transplantation)
- For SCC, other RFs include HPV (types 16, 18, 31, 33, 38), chemical carcinogens
- SCC arise from keratinocytes
- Most BCC arise from epidermal cells differentiated in the primitive hair bulb
Clinical features
- Most develop on head& neck (80%) & are more likely to recur compared to lesions developing on the trunk or extremities
- SCC can arise on mucous membranes, BCC do not
- SCCs appear as hyperkeratotic papules or plaques often w/crust or ulceration
- SCC precursor lesions: Actinic keratosis, cutaneous horn, keratoacanthoma
- Bowen disease: SCC in situ on the skin
- Basosquamous carcinomas: Mixed histology of both BCC & SCC; classified under SCC & risk of met determined by the squamous component

Glute Bridge 10 reps:
Lie flat on your back on a mat or a comfortable floor with your hands by your side. Bend your knees while keeping your feet flat on the floor. Push your hips up and squeeze your glutes to form a 90 degree angle at your knees. Concentrate on squeezing your glutes and hold for a count of 3 at the top.
Spiderman x 6 each side:
Position yourself on the ground in a push-up position with hands shoulder width apart or a little wider, feet together resting on your toes, keeping your spine and head in a neutral position. Holding this position bring one leg up and diagonally forward stepping up as far as you can manage. Keep your butt down and your core strong; use your abdominals to bring your leg forward.
Repeat alternating sides for the desired reps (noting that if your program states 10 repetitions; that is 10 repetitions on each side, for a total of 20 reps)
Step Over x 8 each leg:
(If you have access to a bar or pole to step over, you may use that as a prop) Keep your body upright and raise your leg up and over as you step over (or pretend to step over) a bar/beam.

(Serves 2)
Ingredients:
• 500g ofasparagus,broccoli,or greenbeans;cutandtrimmed
• 1 yellow capsicum, cut in strips
• ½ cup slivered red onion
• ½ cup bean sprouts, rinsed
• 5 tbsp. spicy mustard
• 6 tbsp. balsamic vinegar
• 2 packets of Stevia
• 1 tsp. minced garlic
• 1 tbsp. lemon juice
• 1 tbsp. olive oil or Udo's Oil
• Pepper to taste
Method:
1. Blanch the asparagus in a pot of boiling water - usually about 1 minute; they'll be a nice bright green.
2. Shock it with ice water to stop the cooking process. Toss with the capsicum and onions in a bowl. Set aside.
3. The remaining ingredients will be your dressing. Mix them together in a separate bowl and drizzle on top. Garnish with the sprouts.
*Serve warm or cold. Keeps about 4 days in the fridge. This also works well with green beans or broccoliinstead of asparagus- whatever isin season!!

Ingredientes:
• 500g de Espárrago, brócoli o judías verdes, cortados
• 1 pimentón amarillo, cortado en tiras

• 5cdas. de mostaza picante
• 6cdas. de vinagre balsámico
• 2 paquetes de stevia
• 1cdta. de ajo picado
• 1cda. de jugo de limón
• 1cda. de aceite de oliva o Aceite de Udo
• Pimienta al gusto
Preparación:
1. Blanquea los espárragos en una olla de agua hirviendo – usualmente alrededor de 1 minuto; y tendrán un buen color verde brillante
2. Añade agua fría para detener el proceso de cocina. Colócalo con el pimentón y la cebolla en una taza. Colócalo aparte
3. Los ingredientes restantes serán tu aderezo. Mézclalos en una taza aparte y rocíalo encima. Adorna con la soya
*Sirve tibio o frío. Mantenlo alrededor de 4 días en la nevera. Esto también funciona bien con judías verdes o brócoli en lugar de espárragos – lo que esté en temporada!!
