- Steroid dependant vs frequent relapsing Nephrotic Syndrome.
- Current relapse of nephrotic syndrome
-Acute kidney injury, secondary to dehydration vs Intravscular depletion, with rapid progressive coarse.
Jazi is a ll-؛-sar- old female, known case, of nephrotic syndrome , diagnosed at age of 4 years , with frequent relapse vs steroid dependent (uncertain history as she was managed at MCH-Dammam) . she was treated for a period of 3 years with steroids and a coarse of oral cyclophosphamide. No renal biopsy done at that time. The last relapse was about one year bacJc, and she was treated with oral steroids, in addition to Levamisole . she stopped the steroid about 3 months ago, and Levamisole 1 week ago.
she was in her usual state of health, until 4 days prior to her current admission , when she developed fever, and flu -like symptoms with sore throat. She also had generalized abdomen pain with vomiting .The mother noted decrease-in her urine amount , with frothy appearance . She had one attack of fainting on the day of presentation with no loss of consciousness.
On examination: school aged girl, conscious, oriented, afebrile ,
not in respiratory distress, with negative meningeal signs, vital signs on presentation showed: BP=120/82 , PULSE =168, RR = 22 ,
Tem=36.1 .
She was mildly dehydrated. There was no periorbital edema. Abdomen was soft not tender with no ascites. Chest was clear to auscultation Sc no lower limb edema. (wt 49 kg, Ht 145)
The initial investigation showed: RFT : EUN= 8 , Creat= 0.8 , K=4.8 ,
Na= 140 .
Albumin 1.7 g/dl
CBC: WBC= 18.5, Hb 17.2, Hct 50.8, Pit 451 Urine protein/ Urine creatinine ratio =2.6

- تعاني المريضة من مرض يعتمد على الأسترويد في مقابل انتكاسات متكررة في المتلازمة الكلوية(التناذر)
- تبين وجود انتكاسة حالية في المتلازمة الكلوية
واصابة كلوية حادة نتيجة ثانوية للجفاف ونزح في المنطقة المحيطة بالحجاج مع جفاف متطور بشكل سريع.
عرفت حالة المريضة جازل صاحبة الإحدى عشر عاما بانتكاسات متكررة في المتلازمة الكلوية وشخصت هذه الحالة وهي في عمر الرابعة بوجود انتكاسات متكررة في ظل اعتمادها على الأسترويد. (ويعد التاريخ المرضي لها غير مؤكد حيث خضعت للإجراءات العلاجية بمستشفى الدمام. وتم علاجها لمدة 3 سنوات بالأسترويد ودورة علاجية من السيكلوفوسفاميد الفموي. ولم يجرى لها خزعة او يؤخذ منها عينة في ذلك الوقت. وكانت أخر انتكاسة لها منذ عام وخضعت حينها لاسترويد فموي اضافة إلى عقار الليمفاسول. وأقلعت عن تناول الأسترويد منذ 3 أشهر وعن عقار الليمفاسول منذ ما يقرب من اسبوع تقريبا.
كانت المريضة في حالتها الصحية الطبيعية حتى أربعة أيام قبل ايداعها الحالي بالمستشفى حيث تطورت الحالة إلى حمى وبرد وأعراض مشابهه لاحتقان في الحلق وتطورت الحالة معها إلى الاّم في البطن وقيئ. ولاحظت الأم انخفاض في كمية البول لدى المريضة بنسبة 40%تقريبا. وتعرضت المريضة لحالة اغماء في يوم عرضها على المستشفى دون فقد للوعي.
الكشوفات الأولية: طفلة في عمر الدراسة، مدركة، واعية، غير محمومة.
لم تكن في ضائقة تنفسية، خالية من الأمراض السحائية وأظهرت الكشوفات على العلامات الحيوية النتائج التالية: قياس ضغط الدم 120/82، قياس النبض 168، معدل التنفس 22.قياس درجةالحرارة36.1.
وكانت المريضة تشعر ايضا بجفاف نوعا ما ولم يسجل وجود لتورم في المنطقة المحيطة بالحجاج. كشوفات البطن: تبين بالكشف الباطني على المريضة انها لا تعاني من ثمة تورم أو تحجر في البطن أو استسقاء. وبالكشف الصدري عليها كان صوت النفس واضحا نقيا عند التسمع كما لم يسجل وجود لتورم سفلي في الأطراف وتبين ايضا أن وزن المريضة 49 كيلوغرام، وطولها 145 سم.
أوضحت الفحوصات الاولية النتائج الأتية: تحليل وظائف الكبد: نسبة اليوريا في الدم 8، نسبة البوتاسيوم في الدم 4.8
نسبة الصوديوم 140
نسبة الالبومين 1.7 غرام/ ديسيلتر
تحليل صورة الدم: كرات الدم البيضاء18.5، نسبة الهيموجلوبين 17.2، نسبة الهيماتوكريت (الخلايا المكدسة) 50.8، نسبة الصفائح الدموية 451، نسبة البروتين في البول / نسبة الكرياتينين في البول 2.6.
