global developmental delay
History of Present Illness
Benayan attended with Parents for a review assessment.
Parents explained that he is making progress. His language is improving but he has not yet started speech and language Therapy.
Parents request a referral and advice on where to go. School: Benayan is not yet attending school.
Father explained he would prefer Benayan to attends a specialist center since he can be reimbursed by his employer.
Parents feel that his interaction with brothers is much better but he can still be shy with other children if he does not know them well.
His eye contact is still inconsistent.
Benayan has difficulty conveying his wishes. he may hit his brother when he wants to play with him rather than ask him.
Benayan's only obsession is the mobile. He watches cartoons and he often copies what they are saying. He will watch the same video over and over again. One of his favorites is Ben Ten and he likes to buy toys with ben Ten pictures.
He occasionally plays with toys, mainly a sword to imitate fights.
Observation
Today Benayan behaved very much like last time. He clams up, stays close to his Father, eyes down and frowns constantly. If requested to look or handle a toy, he looks the other way and repeats a very determined "la,la,la" gesturing with his arm and hand stretched to avoid anyone coming near him.
He was shown a variety of toys but showed no interest at all and remained aloof the whole time.
It was not possible to perform a valid assessment.
Conclusion
Benayan is now 6 years of age and presents with Language and Global Developmenta Delay the extent of which has not been possible to assess accurately. He is not in school yet.
Parents are keen to place in a specialist center and we provided them with a list of possible centers. They are not interested in attending Rumailah for SLT and prefer to have private sessions.
We discussed again possible genetic testing and Parents agreed. This will be done next week when Parents will come to get the report since Benayan was very upset today.
We will prepare a comprehensive report for parents which they can present to the Center. Once Benayan attends for some time, we will liaise with colleagues at the Center for a better understanding of his performance and level of delay.
Benayan has been seen by dentist and is waiting treatment under GA
We will review Benayan again in 6 months.

مَثُل إلينا الطفل بِنايان برفقة والديه لمراجعة تقييم حالته الصحية.
أوضح الوالدان تقدُّم حالته. فلغته قد تحسنت إلُّا أنه لم يخضع للتقويم العلاجيّ للتخاطب واللغة.
طلب الوالدان إذن إحالة لجهة أخرى وكذلك النصيحة لماهية هذه الجهة. المدرسة: لم يتمكن الطفل بِنايان من الحضور للمدرسة بعد.
وكشف والده عن رغبته لنقله لمركزٍ متخصصٍ حيث أنه سيتمكن من استرداد تلك النفقات من ربِّ عملِه.
لمس الوالدان تحسناً فى تفاعله مع إخوته إلَّا أنه مازال خجولاً مع الأطفال الآخرين طالما يفتقر لمعرفتهم الجيدة.
ولا زال تواصله البصريّ غير متوافق.
ويعاني بِنايان من صعوبةٍ فى إيصال رغباته. فقد يضرب أخاه عندما يريد اللعب معه بدلاً من أن يطلب منه ذلك.
ويُعَّد الهاتف الجوال هو وسواسه الوحيد. فهو يشاهد أفلام الرسوم المتحركة وينسخ ما يقولون. ولا يمل من مشاهدة نفس الفيديو مراراً وتكراراً. ويعتبر الفيلم بِن تِن الكارتونيّ من أحد أحب أفلامه حتى أنه يحب شراء الدُّمى ذات صورة بِن تِن.
ومن حين لآخر يلعب بِدُمَّاه, خاصةً السيف ليحاكي المشاجرات.
الملاحظة
يسلُك بِنايان اليوم سلوكاً شبيهاً بالزيارة السابقة. فهو مطبقٌ شفتيه, جالسٌ بالقرب من والده, ينظر للأرض ودائم التجهم. وحينما يُطلَب منه الإمساك بدُمية, يُشيح بوجهه جانباً ويردد بشكل قاطع "لا,لا,لا" مُمِيئاً بذراعه باسطاً يده ليمنع أياً كان من الاقتراب.
عرضنا عليه العديد من الدُمى المتنوعة لكنه لم يبالي على الإطلاق وظل مُتحفِّظاً طول الوقت.
لذا كان من الصعوبة إجراء تقييم دقيق للحالة.
الاستنتاج النهائيّ
يشكو الطفل بِنايان البالغ من العمر حالياً 6 سنوات من تأخر لغويّ ونمائيّ عام إلى الحدّ الذي يُصعِّب من التقييم الدقيق لحالتهِ. ولم يلتحق بالمدرسة بعد.
أبدى الوالدان رغبتهما بوضع الطفل بمركزٍ متخصصٍ وقد أمددناهما بقائمة المراكز المتاحة. ولم يكترث الوالدان بالذهاب لمستشفى الرُّمَيلة للعلاج اللُّغويّ والتخاطب وفضلوا الجلسات العِلاجية الخاصة.
وعاودنا مناقشة الاختبارات الچينية المحتملة وقد وافق الوالدان. وسيتم الخضوع إليها الأسبوع المقبل حين الحضور لاستلام التقرير نظراً لانزعاج بِنايان اليوم.
سنجهز تقرير طبيّ متكامل للوالدين لتقديمه للمركز الطبيّ. ووقت حضور الطفل بِنايان لبعض الوقت,سنتصل بالزملاء بالمركز لضمان فهمٍ أفضلٍ لأدائه وتقييم درجة تأخره.
رُوجِع بِنايان بواسطة طبيب الأسنان منتظراً المعالجة تحت تأثير التخدير العام وسنراجع الطفل ثانيةً فى ال6 أشهر المقبلين.

Writing about medical issues is often depressing. What we talk about most often are subjects like disease, human suffering, and death, not pleasant topics at all. And yet for us as clinicians there is much that is worth feeling good about. Starting my career, a young man came to our hospital’s surgical clinic complaining of severe pain in his nether regions. He had suffered mightily for days and finally when he could not take it anymore he overcame his embarrassment and turned up for treatment. All that needed to be done was a swab and a spritz of antiseptic paint, a spray of local anesthesia and a quick ‘cruciate’ (cross like) incision with a lancet. The infection was let out and with it the pain. In about five minutes, a smiling patient was thanking us profusely with a smile that literally ran from ear to ear. Regarding translation, it is a great feeling to know that I could be the person between the doctors and the poor people who have so many questions and who need to understand what is going on, which can not only be translated into letters and words but also into lovely smiles on suffering faces. It is the turning point since when I decided with great honor not only to practice but also to spread medical sciences among the different cultures. Because decoding a message and conveying it in another language is challenging and the results will be extremely rewarding for the patient. Bringing medicine into the recipe makes my job all the more fascinating.
