Explanation:
according to me in form and substance is the expression sanctioned by use in "legalese"
the second one has the same meaning and it is more descriptive, stressing the physical existence of a form "document" guess the translator had a problem with the bisemantic meaning of "form",so he used the adverb "substantially"
hope it helps
you won't have the same problem in spanish (two different words)
I would maybe use the first solution when translating into spanish
