1. Follow normal accepted practice for vessel puncture and guidewire insertion, vessel dilator and introducer use.
2. Advance the dilator sheath introducer assembly over the exposed guidewire into the vessel, using a twisting motion.
CAUTION: DO NOT ATTEMPT ADVANCEMENT WITHOUT GUIDEWIRE. SEVERE VASCULAR DAMAGE AND/OR INJURY MAY OCCUR.

3. Withdraw the vessel dilator and guidewire, leaving the introducer sheath in position. Place thumb over the exposed orifice of the sheath to reduce air aspiration and/or blood loss.
4. Attach SJM FAST-CATH DUO™ or SJM FAST-CATH TRIO™ hemostasis adapter to introducer sheath.
CAUTION: ASPIRATE THROUGH ALL SIDEPORTS OF THE SJM FAST-CATH DUO™ OR SJM FAST-CATH TRIO™ HEMOSTASIS ADAPTER, INTRODUCER SHEATH ASSEMBLY TO REMOVE ALL AIR PRIOR TO INSERTION OF A DEVICE.
5. Follow manufacturer's recommendations for the catheter or device being introduced via the hemostasis introducer.
6. Follow normal practice of using a continuous drip of anticoagulant fluid under pressure through the sideports when the hemostasis adapter/introducer assembly is in the vessel.

Source text - English A limited liability company or a company with limited liability (abbreviated L.L.C. or LLC or W.L.L) in the law of the vast majority of United States jurisdictions is a legal form of business company that provides limited liability to its owners. Often incorrectly called a "limited liability corporation" (instead of company), it is a hybrid business entity having certain characteristics of both a corporation and a partnership or sole proprietorship (depending on how many owners there are). An LLC, although a business entity, is a type of unincorporated association and is not a corporation. The primary characteristic an LLC shares with a corporation is limited liability, and the primary characteristic it shares with a partnership is the availability of pass-through income taxation. It is often more flexible than a corporation and it is well-suited for companies with a single owner.
It is important to understand that limited liability does not imply owners are always fully protected from personal liabilities. Courts can and do pierce the corporate veil of LLCs when some type of fraud or misrepresentation is involved, or under certain situations where the owner uses the company as an "alter ego."" gtc:suffix="" gtc:mediawiki-xid="2"[2]
Terminology Member LLC members are the owners of the LLC as much as shareholders are the owners of a corporation or the partners of a partnership. Like shareholders, a member's liability to repay the LLC's obligations is limited to his or her capital contribution. Members may be natural persons, corporations, partnerships, or other LLCs.
Membership Interest A member's ownership interest in an LLC is often called a membership interest. Membership interests are often divided into standardized units which, in turn, are often called shares or units. Unless otherwise provided for in the operating agreement, a member's right to receive distributions or exercise member rights over the LLC is proportionate to their membership interest. Membership interests and member rights are regulated by state law.
Manager In most states, LLCs may be managed by their members in proportion to their membership interests. In some states such as Georgia, however, each member has an equal right to participate in the management of the LLC unless there is a specific provision in the articles of organization or operating agreement to the contrary. Many LLC operating agreements, however, provide for a manager or board of managers to oversee or run the day-to-day operations of the LLC. The managers are elected or appointed by members and may also be, if so provided in the operating agreement, removed by members. A member may also be a manager. There is some confusion as to how to treat someone who is identified as the "managing member" of an LLC. Although suggestions have been made on earlier versions of this entry that a "managing member" should be treated as a manager, the better treatment is to consider a "managing member" as being a member.
Articles of Organization All LLCs must file a document to form the LLC with the secretary of state (or other specified governmental office) of the state where the persons who form the LLC choose to organize it. This document is usually referrd to as "Articles of Organization" or "Certificate of Organization," depending on the state. The LLC may be formed in the state where the organizers are located, where the LLC will conduct business, or in another state.
Operating Agreement It is common for an LLC to have an "operating agreement" which defines and apportions the rights of the members and the managers, if any, including issues of membership, management, operation, and distribution of income of the LLC. The operating agreement generally is not filed with the secretary of state (or other governmental office). The operating agreement is a contract among the members, the managers, and the LLC.
Flexibility and default rulesThe phrase "unless otherwise provided for in the operating agreement" (or its equivalent) is found throughout all existing LLC statutes and is responsible for the flexibility the members of the LLC have in deciding how their LLC will be governed (provided it does not go outside legal bounds). State statutes typically provide automatic or "default" rules for how an LLC will be governed unless the Operating Agreement provides otherwise.
Similarly, the phrase "unless otherwise provided for in the bylaws" is also found in all corporation law statutes but often refers only to a narrower range of matters.
ManagementLLCs may be either member-managed or manager-managed. A member-managed LLC may be governed by a single class of members (in which case it approximates a partnership) or multiple classes of members (in which case it approximates a limited partnership). Choosing manager management creates a two-tiered management structure that approximates corporate governance with the managers typically holding powers similar to corporate officers and directors. The LLC's operating agreement (the LLC version of a partnership agreement or a corporation's bylaws) determines how the LLC is managed. Corporations, S-corporations, Limited Liability Partnerships, Limited Partnerships, Limited Liability Limited Partnerships, and LLCs lie along a spectrum of flexibility with LLCs being the most flexible, and thus preferable, for many businesses.
Income taxationFor U.S. Federal income tax purposes, LLCs are treated by default as a pass-through entity" gtc:suffix="" gtc:mediawiki-xid="3"[3]. If there is only one member, it is treated as a "disregarded entity" for tax purposes, and the owner reports the LLC's income on his or her own tax return on Schedule C. For LLCs with multiple members, the LLC is treated as a partnership and must file IRS Form 1065. Individual partners would receive a K-1 for their share of income or losses to be reported on that owner's tax return.
As an option, LLCs may also elect to be taxed like a corporation by filing IRS Form 8832" gtc:suffix="" gtc:mediawiki-xid="4"[4]. They can be treated as a regular C-corporation (taxation of the entity's income prior to any dividends or distributions to the members and then taxation of the dividends or distributions once received as income by the members), or an LLC can elect to be treated as an S-corporation. Some commentators have recommended an LLC taxed as a S-corporation as the best possible small business structure. It combines the simplicity and flexibility of an LLC with the tax benefits of an S-corporation (self-employment tax savings)" gtc:suffix="" gtc:mediawiki-xid="5"[5].
Advantages
Check-the-box taxation. An LLC can elect to be taxed as a sole proprietor, partnership, S corporation or C corporation (as long as they would otherwise qualify for such tax treatment), providing much flexibility.
Limited liability, meaning that the owners of the LLC, called "members," are protected from some or all liability for acts and debts of the LLC depending on state shield laws.
Much less administrative paperwork and record keeping than a corporation.
Pass-through taxation (i.e., no double taxation), unless the LLC elects to be taxed as a C corporation.
Using default tax classification, profits are taxed personally at the member level, not at the LLC level.
LLCs in most states are treated as entities separate from their members, whereas in other jurisdictions[6] case law has developed deciding LLCs are not considered to have separate legal standing from their members (see recent D.C. decisions[7]).
LLCs in some states can be set up with just one natural person involved.
Membership interests of LLCs can be assigned, and the economic benefits of those interests can be separated and assigned, providing the assignee with the economic benefits of distributions of profits/losses (like a partnership), without transferring the title to the membership interest (see, for example, the Virginia and Delaware LLC Acts).

In life, there are many unplanned events that force us to dip into our savings or take loans. A sudden illness, a death in the family, a natural calamity or other events that were quite unexpected can be quite catastrophic as they require large amounts of money to pay for the related expenses. Social protection programmes, like insurances, help keep us from being made vulnerable by these events. This module introduces participants to the basics of insurance programmes.

1. Understand how insurance programmes and products provide social protection and appreciate the importance of them in their lives
2. Identify the different kinds of insurance products
3. Practice going through a benefit package and the criteria in selection

=== 1 Explore ===
Activity: Treasure Pot (30 minutes)
This is a game that simulates emergency health situations and the need for medical insurance. Be sure to read through the instructions before doing the game.
Materials
» Game cards (to be prepared as described below)
» Stones or beans or candies (7 per person)
» 3 bowls or boxes
Note: Prepare the right number of cards for the number of participants. Prepare extra cards if the exact number is unknown. All cards should look identical if reversed; when folded, it should not be possible to recognize specific cards. The table below indicates the number of “Event” cards (Drugs, Hospital, Doctor, Healthy) required for the number of participants. It is important to adhere to the number of respective event cards as these are calculated to fit with the participant’s budget.
Process
» Have all participants sit in a circle so they can see each other.
» Mix the folded ‘event cards’ and place them in one of the bowls (‘events bowl’) on a central display table.
ROUND ONE
» Begin handing out 7 stones/beans to each person and begin the explanation:
You have received 7 stones/beans that symbolize the funds you have. For instance, you can use these funds for healthcare. Please go to the table and pick one card each from the bowl. This card states your health status. If you’re fortunate, you remain healthy and you have no health-related expenses. However, over a year, it is quite rare not to have any health related expenses. For example, you might have to purchase drugs or you might have to visit the doctor. The costs of these will be indicated on the card you pick. A worst case scenario would be hospitalization for which the cost is substantial. This might not be as frequent as buying drugs or visiting the doctor. Nevertheless, these costs accumulate over time.
» Ask participants to walk to the display table one at a time, announce the health related expense that they had and pay the costs if needed. Pay the amount to another bowl kept on the table.
» Those who picked the hospitalization card are already out of funds. Announce this to everyone else in the group.
ROUND TWO
» Collect all the cards and return them to the ‘events bowl’. Repeat round one. When people run out of funds again, it is important to draw attention to the need for insurance.
» Pose some questions:
What were the reasons why people ran out of money?
Do you experience this in real life? What do people usually do in these situations?
» Explanation: Let’s draw attention to these concepts: Poverty! Debt! Selling assets! Limited possibilities! No more access to care! This is the situation we face without insurance. This is how life looks like for most people in our community. It is very important to plan, as you never know when you need to spend on health. Sometimes, the expenses turn out to be quite substantial and this pulls people into poverty, forcing them to sell their assets, or forego seeking necessary health care. This leaves us unnecessarily vulnerable.
STORYTELLING
» Read out the story attached “The Crow”
There is an ancient story about a crow that drank water from the bottom of a pot. A crow became thirsty after flying around a lot and decided to look for water. It looked in different places but was not successful. Finally, the crow saw a pot which had some water at the bottom of it. But the crow was unable to get deep enough into the pot to reach the water! It could see the water but could not get it. The intelligent crow picked small pebbles from around the pot and threw it in. The water level continued to rise and the crow continued to throw pebbles into the water until it was closer to the crow to drink it. The crow was able to quench its thirst because it did not give up and found an intelligent solution to its problem.
The story teaches us a valuable lesson. For the majority of poor people, health services are like the water in the story. They can see that the service exists but they cannot reach it. We saw that at the end of the game, a number of people had no funds left to pay for all the services needed. Let’s be smart and find a way to make health care more accessible to all.
ROUND THREE
» Repeat the game with the same cards and the same budget for each person (7 stones/ beans).
» Before getting event cards, explain that you will introduce a new step. Ask a volunteer to collect three stones/beans from everyone and put them in the 2nd bowl. Explain that [the volunteer] is the community insurance officer. The bowl he is collecting is our “Treasure pot” or our insurance fund that will cover for all health costs.
» Now ask two volunteers to act as the “claims committee”. They are responsible for collecting the payments and administrating the money from the fund. If someone gets ill, they will make the payment that is needed. They will stand next to the fund and make payments if needed.
» Ask participants to get one event card each again. Read the contents aloud and the related health expenses. The claims committee will pay out the expenses from the treasure pot.
» Repeat this process until all claims have been cleared.
» At the end of the round, each event will have been paid and all funds will have been used (the claim committee also participates).
» The funds collected were sufficient to pay for all health costs of the group.
Instructions for facilitator
» Be sure to emphasize that the funds collected (treasure pot) were sufficient to pay for all health costs of the group. Refer to the story of the crow, where we were able to bring health care closer to the community by throwing stones into the pot.
» Some may think: “I had more funds left in the first part of the game, without insurance”. Emphasize that we had only played a few rounds and that things could change very quickly for different people. Having insurance guarantees that everyone is safe and protected from health risks.

The simulation game showed elements of how we are vulnerable to health risks and how a social protection programme like insurance can help make us less vulnerable.

» Form small groups with 4 participants in each group.
» Health insurance is one form of insurance. It protects against unexpected sickness or medical emergencies.
» Ask them to think of other forms of unexpected emergencies that can make them in need of money (financially vulnerable).
» For each of these emergencies, write down what kind of insurance package has been developed for it.
» Ask the groups to present their lists.

» It may be good to have an overview of the different insurance programmes available in your community (including community micro-insurance programmes).

KINDS of INSURANCE
» Auto insurance - Auto insurance protects you against financial loss if you have a car or road accident.
» Home insurance - Home insurance provides compensation for damage or destruction of a home from disasters. In some geographical areas, the standard insurances exclude certain types of disasters, such as flood and earthquakes. Those require additional coverage.
» Health - Health insurance covers the cost of medical treatments
» Accident, Sickness and Unemployment Insurance - provide financial support in the event the policyholder is unable to work because of disabling illness or injury.
» Casualty - insures against accidents, not necessarily tied to any specific property.
» Life - monetary benefit to a decedent's family or other designated beneficiary, and may specifically provide for income to an insured person's family, burial, funeral and other final expenses
» Property - provides protection against risks to property, such as fire, theft or weather damage. This includes specialized forms of insurance such as fire insurance, flood insurance, earthquake insurance, home insurance, inland marine insurance or boiler insurance.
» Liability - The protection offered by a liability insurance policy is twofold: a legal defence in the event of a lawsuit commenced against the policyholder and indemnification (payment on behalf of the insured) with respect to a settlement or court verdict.
» Credit - repays some or all of a loan when certain things happen to the borrower such as unemployment, disability, or death.
» Other types – Pet insurance, kidnap insurance, among others
» Micro-insurance – communities or self-help groups develop an informal way of providing insurance support within their groups, especially when formal insurance systems are not accessible.
THINGS to CONSIDER when choosing an INSURANCE
» Premium and method of payment – How much is the package? How often would you pay? How will the payment be collected?
» Claims system – Is it a reimbursement system? Or will hospitals and clinics get the payment directly from the insurance?
» Benefits covered – Does it cover most of your medical needs? Are there some medical conditions that it does not cover which are important to you? Does it cover other people in your family? Can you use it outside your town or city?
» Criteria for membership – Is it easy to become a member? What are the requirements that you need to provide?

After all that has been learned, participants can put into practice what they have learned by choosing an insurance package (whether as an exercise or in actuality).
Activity: Choosing a Benefit Package (20 minutes)
Participants will be asked to go through several health insurance benefit packages and choose one.

» 3 different samples of benefit packages (actual benefit packages)
Process
» In the same groups, have the participants read through the 3 different examples of benefit packages.
» Ask them to choose a benefit package for themselves.
» Have each group report their choice and give their reasons.
Instructions for facilitator
» It would be very helpful if you could collect sample benefit packages from actual insurance programmes that are in your community.
» Try to deepen the discussion by talking about whether the existing insurance programmes are accessible to children, youth, poor families or the elderly; and re-introduce MICRO-INSURANCE as one of the initiatives for this.

» التأمين على السيارات -- التأمين على السيارات يحميك ضد الخسارة المالية إذا كان لديك سيارة أو حادث سير.
» التأمين المنزلي -- التأمين المنزلي يوفر التعويض عن الضرر أو تدمير المنزل من جراء الكوارث. في بعض المناطق الجغرافية ، يقوم التأمين الإعتيادي باستبعاد أنواع معينة من الكوارث، مثل الفيضانات والزلازل. فإن هذه تحتاج إلى تغطية إضافية.

» التأمين المصغّر -- المجتمعات أو مجموعات المساعدة الذاتية وضعت طريقة غير رسمية لتقديم دعم التأمين داخل مجموعاتهم، وخاصة عندما تكن نظم التأمين الرسمي لا يمكن الوصول إليها.
أمور يجب النظر إليها عند إختيار التأمين
» القيمة وطريقة الدفع -- كم سعر الحزمة؟ ما المدة التى سيتم فيها الدفع؟ كيف سيتم جمع المدفوعات؟
» نظام المطالبات -- هل هو بنظام التعويض؟ أم أن المستشفيات والعيادات ستحصل على المبالغ مباشرة من التأمين؟
» المنافع المغطاة -- هل تغطي معظم الاحتياجات الطبية الخاصة بك؟ هل هناك بعض الحالات الطبية التي لا تغطي والتي تعتبر هامة بالنسبة لك؟ هل يغطي الأفراد الآخرين في عائلتك؟ هل يمكنك استخدامه خارج البلدة أو المدينة؟
» معايير العضوية -- هل من السهل أن تصبح عضوا؟ ما هي المتطلبات التي تحتاج إلى توفيرها؟

بعد كل ما تم تعلمه ، يمكن للمشاركين وضع ما تعلموه موضع التنفيذ عن طريق اختيار حزمة التأمين (سواء بصفته ممارسة أو في واقع الأمر).

سيطلب من المشاركين المرور خلال مختلف مجموعات منافع التأمين الصحية واختيار احدهم.

» 3 عينات مختلفة من حزم المنافع (حزم الاستفادة الفعلية)
النهج
» في نفس المجموعات ، اجعل المشاركين يقرأون 3 أمثلة مختلفة من حزم المنافع.
» اطلب منهم اختيار حزمة منافع لأنفسهم.
» اجعل كل مجموعة تقرر اختيارها وتبدي أسبابها.

» سيكون من المفيد جدا لو أمكن جمع عينة من حزم الاستفادة من برامج التأمين الفعلية التي تتوافر في مجتمعك.
» حاول تعميق النقاش من خلال الحديث حول ما إذا كانت برامج التأمين الموجودة هي في متناول الأطفال والشباب والأسر الفقيرة أو المسنين، وأعد عرض التأمين المصغّر باعتباره واحدا من المبادرات لهذا الغرض.
