Translation - English Night Work and Shift Work Night work and successive teams, or shift work, have been adopted to meet needs of continuous production. In industrialised countries they account for about 20% of the active population. This type of work imposes rhythms on the workers that are contradictory to their entourage and their own biological rhythms. This shift pattern promotes the emergence of a range of disorders that often make life difficult, and which, in 10 years' time and 30% of cases, require the night worker to temporarily or permanently cease activity. Shift work-related disorders In addition to social and family related difficulties, night work and shift work upset the biological rhythms of the body resulting in a series of disorders. The main disorders are those related to sleep. • drowsiness and decreased alertness in the second part of the night • difficulty in falling asleep • insufficient and non-restorative sleep causing chronic fatigue These sleep disorders promote the onset of mental disorders and trigger various chronic diseases. These chronic diseases generally appear after several years' work. Sleep disorders and chronic fatigue: 70% Productivity, performance and alertness at work:-10% Family problems: 10 to 15% Decline in the quality of life: 20 to 30% Mood swings and depressive tendencies: 15-20% Cardiovascular disorders: x 2 Gastric disorders: x 5 Sleep linked disorders are more marked among workers on a flexible work schedule and become increasingly significant with the length of rotations (8 days). Short rotations (2-3 days) appear more favourable since they leave no time for the body to change its circadian rhythms (24 hour rhythms). Shift work disorder affects all age groups and both sexes. However, their frequency increases with age. The consequences of these disorders are important security and economic levels. They can be demonstrated by decrease in production yields and increase in the rate of absenteeism and frequency of work errors (dangerous or not). Biological rhythms and regulation Most human beings' biological parameters fluctuate on a 24 hour cycle. These are circadian rhythms. The primary circadian rhythm is alternating sleep/wake. The alternation of these rhythms is influenced by external synchronizers, the major one being alternating light/dark. Light regulates the production of melatonin, commonly called "the sleep hormone". The supply of light blocks the production of melatonin, which promotes arousal. As night approaches, the level of blood melatonin increases, which promotes the induction of sleep. When our internal clock is no longer synchronized with the actual clock, a pathological situation ensues. This is the case of the night worker. His work rhythm forces him to sleep when day breaks and his rate of melatonin decreases and vice versa. The night worker lives in constant conflict to nature’s intention. Light therapy and night work Some of the recommendations related to light can help the worker adapt his body to night work. 1. Thanks to its inhibitory effect of melatonin secretion, the supply of adequate light at an appropriate moment enables the resynchronisation of the wake/sleep cycle and improves the quality of sleep and the individual's alertness. 2. Conversely, the use of filter glasses prevents blocking melatonin and allows easier access to sleep. 3. Other so-called chrono-therapeutic tools can influence the regulation of the biological clock. (physical exercise - hot baths and intake of specific foods, etc.) In light therapy, only the blue portion of the visible spectrum, between 430 and 500 nm, is active and enables influence on biological rhythms and particularly the sleep/wake rhythm. Similarly, by preventing the blue light from entering the eye using filter glasses, it is possible to influence these biological rhythms. The timing of the use of light or light deprivation is crucial, because it will enable advancing or delaying the internal biological clock, thereby causing either a phase advance or delay. La luminette ®, result of academic research, is currently the most effective light therapy device. A sophisticated optical device (hologram) allows the light to be sent to the right place in the retina without altering vision. Lightweight (63 gr), it is worn as glasses and does not dazzle. La luminette ® is the only light therapy device that allows you to carry out your session without interrupting your work. Resynchronisation programme CHRONO SleepWell ® is a chronotherapy software using algorithms that enables the generation of customised realignment programmes. It is the culmination of a long experience acquired with jet-lagged travellers and patients with circadian disorders, for whom the proponent has designed numerous realignment programmes in the last 15 years. °These internal biological clock resynchronisation programmes integrate all known chrono-therapeutic tools (exposure to light - avoidance of daylight - intake of melatonin - physical exercise - the drop in body temperature, etc.). Attached you will find various tables that correspond to different work rhythms. It is strongly advised to follow, at least, the main recommendations.
