reminding me and many thanks for getting back to me so quickly. Yes 'storia di' does make more sense. So far I have translated as 'patient underwent' followed by whatever surgical procedure and the year, which seems to fit my particular context, so will keep 'history of' for future reference. Thanks again for your help :)

I don't remember what I used in this translation ... and at this point, when I look into this again, I'm still not 100% sure. In ACROMED I find this as "Storia di". In another online search I just did, I find a document that uses it to mean "Stato dopo" ... see: http://memcms.memdoc.org/fileadmin/memdoc/pdf/siris_ides_kne... (in the section titled Ricovero Aggiuntiva) ... but this one has a few mistakes in it, and I would trust it less. I'd go with what Gilberto chose to use in ACROMED, since he's a practicing physician.

I was wondering if you ever found out if St.d. does actually stand for Stato da?
It comes up repeatedly in the Swiss hospital discharge letter I'm translating and I'm unsure how best to translate it. Did you find any confirmation for St.p. (status post)? Many thanks in advance!

.... is what this stands for, I think. I'm finding many instances of 'stato da' in the series of angiography/angioplasty reports from the same cardiology center that I'm working on. I asked another question about this 'stato da' (versus 'stato dopo') phrase just a day after I posted this one.

Some examples of 'stato da' in the other documents I've been translating:

"Stato da polimialgia reumatica e arterite gigantocellulare."
"Paziente 80enne in stato da PM bicamerale nel 2006." [This is history, not current ... the report was written in 2010.]
"Stato da ematoma del muscolo del retto ..."
