Source text - English As the risk to your unborn child is unknown, we kindly ask you to sign this consent form to allow the collection of data related to your pregnancy and your child/children’s health condition during the pregnancy and the outcome of your pregnancy.
If you sign this document, you are giving permission for the use and disclosure of your/your child’s health information for the purposes of monitoring the potential risk for you or your baby related to your partner’s study medication administration.
You do not have to give this permission. Your participation in this data collection research is voluntary. If you do not agree to have information about your pregnancy collected, this will not affect your partner’s ability to participate in the main research study. Your decision will not affect your, your partner’s, or your baby’s regular medical care.

into your case and decide if we can help you in court. We often use these
forms to learn more about your building and your court case but may use it
to get other kinds of information.
2) Consent to Release Information: This lets us give information to city
agencies so we can get paid by them for the work we do on your case. You
do not need to sign this form. Signing this form will not affect whether we
can help you in court. You do not have to re-pay any money we are paid by
the city for helping you with your case. Our services are always free.
3) Declaration of Household Composition and Income: This form asks
about who lives in your household. We need this information in order to
open a case for you.
4) Citizenship statement: The Legal Services Corporation requires us to get
information about our clients’ citizenship or immigration status. We
represent both citizens and immigrants. We will not share information
about your immigration status with anyone unless you agree to it.
