and means in this case, as Tina Vonhof indicated, no prior surgeries


You can't use OR
'OK' has been used metonymically here and a literal translation, I expect, won't work.

I think it means that the patient has not had any surgeries in the past.

Thanks - I should have mentioned OR had crossed my mind in my brain-wracking but it's too early in the morning. This may have to be an "ask the client" issue.

Do not have context, but the first thing that springs to mind is OK=Operatiekamer and therefore OR (Operating Room), also called operating theatre. Could this fit with your text?
