Source text - English Breast cancer is a worldwide health concern with approximately 1 million new cases each year. Significant advances have been made in our understanding of this malignancy and several molecular subtypes of breast cancer have been characterized. This molecular understanding has paved the way for the development of new agents that target pathogenic molecular alterations that drive tumor cell growth while sparing patients many of the traditional toxicities associated with chemotherapy. Ubiquitous to all cancer types is abnormal proliferation with dysregulation of normal cell cycle control. For this reason, inhibitors of key cell cycle regulators are attractive targets for novel cancer therapeutics. Successful clinical development of this class of agents, however, will require some understanding of which subgroup of patients will be more likely to benefit from these targeted interventions. Under normal control, the cell cycle functions as a tightly regulated and predictable process consisting of several distinct phases: G0 (quiescence) followed by G1 (pre-DNA synthesis), S (DNA synthesis), G2 (pre-division), and M (cell division). The careful regulation of this system is of fundamental importance, and dysregulation can result in several disease processes including cancer. The progression from G1 to S is a key checkpoint in protecting the cell from abnormal replication. Key to passage through this restriction point is the interaction between the cyclin-dependent kinases (CDKs) and cyclin proteins. CDKs are a subgroup of serine/threonine kinases that play a key role in regulating cell cycle progression by associating with cyclins. Hyperphosphorylation of the retinoblastoma (Rb) gene product pRb is mediated in early G1 by CDK4 and CDK6 interacting with cyclin D1. This results in pRB inactivation and release of transcription factors that allow progression to the S phase. Negative regulators of CDK4/6-cyclin activity include the INK4 family (p16, p15, p18, p19). Several studies have identified alterations of cell cycle regulators in human breast cancer and provide a rationale for a potential therapeutic role for CDK4/6 inhibition in this tumor type. Amplification of the cyclin D1 gene has been identified in approximately 15 to 20% of human breast cancers while overexpression of the protein has been demonstrated in a higher percentage. The prognostic significance of cyclin D1 overexpression is not clear; some studies suggest it is a dominant oncogene associated with poor clinical outcomes, while other studies suggest it is associated with a more indolent, estrogen receptor (ER)-positive phenotype. In addition, studies have associated cyclin D amplification with resistance to tamoxifen. While the interaction between CDK4/6 and cyclin D1 suggests their interdependence, cyclin D1 has been found to function independently of CDK4/6 in supporting proliferation by directly activating ER. Finally, loss of function of pRb has been described in 20 to 35% of breast cancers. The majority of CDK targeted agents to date have not focused on CDK4/6 targeting but rather on CDK1/2 targeting. Consequently, the most advanced agents in development are aimed at these targets. Further, limited data exist regarding the preclinical activity of CDK4/6 inhibitors in breast cancer. PD 0332991 is an orally active potent and highly selective inhibitor of CDK4 and CDK6 kinases, which in low nanomolar concentrations blocks pRb phosphorylation - subsequently inducing G1 arrest in sensitive cell lines. Preclinical studies have demonstrated that PD 0332991 induces G1 arrest in primary bone marrow cells ex vivo and prevents tumor growth in disseminated human myeloma xenografts. Based on the above biology, we hypothesized that there might be a molecular subgroup of human breast cancers that would be dependent on CDK4/6 function and would be likely to respond to this agent. Previous studies have demonstrated that in vitro large-panel analyses of molecularly characterized breast cancer cell lines can offer insight into the molecular heterogeneity of the clinical disease. To identify potential biomarkers of response to PD 0332991 and to assist in patient selection and clinical development, we therefore evaluated the effects of PD 0332991 in a panel of 47 human breast cancer and immortalized breast cell lines growing in vitro.
Translation - Czech Karcinom prsu je celosvětovým zdravotnickým problémem s přibližně 1 milionem nových případů ročně. Signifikantní pokrok byl učiněn v našem porozumění této malignitě a bylo charakterizováno několik molekulárních podtypů karcinomu prsu. Toto porozumění na molekulární úrovni vedlo k vývoji nových experimentálních léků, které cílí na patogenní molekulární změny stimulující růst nádorových buněk, a které zároveň umožňují, aby se pacienti vyhnuli mnoha typickým vedlejším účinkům spojených s chemoterapií. Abnormální proliferace s deregulací kontroly buněčného cyklu je přítomna u všech malignit. Z tohoto důvodu jsou inhibitory klíčových regulátorů buněčného cyklu atraktivními cíli nových protinádorových léků. K úspěšnému klinickému vývoji léků této kategorie bude ale nutné, abychom zjistili, který podtyp pacientů má největší šanci, že využije těchto cílených intervencí. Za normální situace buněčný cyklus pracuje jako silně regulovaný a předvídatelný proces, který má několik typických fází: G0 (fáze klidová), kterou následuje fáze G1 (1. přípravná, před syntézou DNA), S (syntéza DNA), G2 (2. přípravná, před buněčným dělením) a M (mitotická, buněčné dělení). Přesná regulace tohoto procesu je naprosto důležitá a deregulace může vést k několika onemocněním, včetně rakoviny. Postup v buněčném cyklu z fáze G1 do fáze S je důležitým kontrolním bodem, který chrání buňku před abnormální replikací. Interakce mezi cyklin-dependentními kinázami (CDK) a proteiny zvanými cykliny jsou klíčové pro postup přes tento kontrolní bod. CDK proteiny jsou podtypem serinových a treoninových kináz, které díky spojování s cykliny hrají klíčovou roli při regulaci postupu buněčného cyklu. Hyperfosforylace retinoblastomového (Rb) genového produktu (pRb) je zprostředkována v rané G1 fázi pomocí CDK4 a CDK6, které jsou v interakci s cyklinem D1. Tento proces vede k inaktivaci pRb a uvolnění transkripčních faktorů, které umožňují postup do fáze S buněčného cyklu. Mezi negativní regulátory aktivity CDK4/6-cyklinu patří INK4 rodina (p16, p15, p18 a p19). Několik studií objasnilo změny v regulátorech buněčného cyklu u lidského karcinomu prsu a poskytlo podklady pro případné terapeutické využití inhibice CDK4/6 u tohoto typu nádoru. Amplifikace genu cyklinu D1 byla identifikována u přibližně 15-20 procent lidského karcinomu prsu, zatímco nadměrná exprese proteinu byla prokázána u vyššího procenta. Prognostický význam nadměrné exprese cyklinu D1 není jasný. Některé studie naznačují, že je to dominantní onkogen, který je spojený se špatnými klinickými výsledky, zatímco jiné studie naznačují, že je spojený s více indolentním fenotypem estrogen receptor (ER) pozitivním. Na základě těchto biologických poznatků jsme navrhli hypotézu, že může existovat podtyp lidského karcinomu prsu, který by byl závislý na funkci CDK4/6, a který by s velkou pravděpodobností reagoval na tento experimentální lék. Předchozí studie prokázaly, že velké panely molekulárně charakterizovaných tkáňových kultur karcinomu prsu mohou objasnit molekulární heterogenitu nádorů u pacientů. Abychom identifikovali možné biomarkery spojené s terapeutickým účinkem PD 0332991 a tak pomohli s výběrem pacientů a s klinickým vývojem, otestovali jsme účinek PD 0332991 na panelu 47 lidských buněčných linií karcinomů prsu a imortalizovaných mamárních buněk rostoucích in vitro.

Souhrn
Transplantace kostní dřeně a periferních kmenových buněk, přes svůj kurativní potenciál, stále představují pro pacienty významné riziko. Toxicita vysokodávkované chemoterapie je jednou z hlavních příčin peritransplantační mortality. Busulfan je cytostatikum, které je často používanou součástí přípravných režimů před transplantací. Zvolení optimální dávky busulfanu a predikce jeho klinického efektu mohou být velmi obtížné. Absorpce busulfanu z gastrointestinálního traktu a úroveň jeho metabolizmu jsou značně variabilní. Několik studií publikovaných v posledních letech ukázalo, že plazmatické koncentrace busulfanu lépe korelují s klinickým efektem a extramedulární toxicitou způsobenou tímto lékem než aktuální dávka busulfanu podaná pacientovi. Byly navrženy přibližné cílové plazmatické koncentrace busulfanu, při nichž bylo dosaženo optimální rovnováhy mezi požadovaným klinickým efektem a rizikem vážných vedlejších účinků chemoterapie. Byly publikovány téměř dvě desítky chromatografických metod, které umožňují kvantitativní stanovení busulfanu. Udržování určité plazmatické hladiny busulfanu v průběhu celého jeho podávání pomocí opakovaných úprav jeho dávkování může redukovat toxicitu tohoto léku a zlepšit výsledky transplantací kostní dřeně či periferních kmenových buněk. Tato metodika je snadno aplikovatelná, je finančně a personálně nenáročná a vyžaduje přístrojové vybavení, které je obvykle snadno dostupné ve většině laboratoří transplantačních center.
Translation - English HIGH-DOSE BUSULFAN--MONITORING PLASMA LEVELS AND DOSAGE ADJUSTMENTS IN ADULTS
Abstract
Bone marrow and peripheral blood stem cell transplantations, despite their curative potential, still carry significant risks for patients. Toxicity of high-dose chemotherapy is one of the leading causes of peritransplant mortality. Busulfan is a frequently used chemotherapeutic agent in conditioning regimens prior to transplantations. The choice of the optimal busulfan dose, and prediction of its clinical effects, may be very difficult. Absorption of busulfan from the gastrointestinal tract and its metabolism can vary considerably. Several recently published studies showed that busulfan plasma concentrations correlate better with clinical effects and extramedullary toxicity caused by this drug than with the actual dose administered to the patient. Approximate target plasma concentrations of busulfan, which meet the optimal balance between the clinical effect and the risk of severe side effects of chemotherapy, were proposed. Almost twenty chromatographic methods have been published, which make the quantitative measurement of busulfan possible. The maintenance of certain busulfan plasma concentrations during its whole administration, with the help of repeated adjustments of its dosage, can reduce the toxicity caused by this agent, and improve the results of bone marrow or peripheral blood stem cell transplantations. This method is easily applicable, has low financial and personal demands, and its required technical appliances are usually accessible in most transplant center laboratories.
