Source text - Spanish En el Perú, actualmente la enfermedad de Carrión, producida por la Bartonella bacilliformis, es un problema de salud pública (3, 4) y su impacto económico, muy alto (5). Durante la última década, la distribución geográfica de este proceso infeccioso se ha extendido a nuevas áreas de transmisión y ha sido reportado no sólo en las clásicas regiones interandinas sino también en Costa, Sierra y Selva (6). A nivel nacional son catorce los departamentos que notifican casos de enfermedad de Carrión, y en Ancash corresponde el 60 a 70% del total (5, 7).
La enfermedad de Carrión tiene tres formas clínicas de presentación: la forma hemática aguda, la crónica verrucosa y la bacteriemia crónica asintomática(9). La forma hemática aguda, bartonelosis aguda o enfermedad de Carrión en fase aguda (fiebre de La Oroya), es la única que produce mortalidad y se caracteriza porque luego de un período de incubación de 7 a 210 días se presenta con fiebre, anemia severa, cefalea, dolor musculoesquelético, ictericia, astenia, hepatoesplenomegalia y cambios en el estado mental relacionado a un estado de inmunosupresión transitoria. Los pacientes se encuentran bacteriémicos y el parasitismo puede alcanzar al 100% de eritrocitos.
Translation - English Currently, Carrion’s disease, caused by Bartonella bacilliformis, represents a public health problem (3,4) and has a strong economic impact (5) on Peru. In the last decade this communicable disease has spread geographically to new transmission areas and been reported not only in the prototypical interandean zones but also on the Coast, in the northern Andes Mountains, and in the Amazon as well (6). Nationally, fourteen regions have reported cases of Carrion’s disease, from which Ancash, located approximately 250 miles north of the capital Lima, accounts for 60 to 70% of the total (5,7).
Carrion’s disease has three clinical phases: acute hematic, verrucous chronic, and chronic asymptomatic bacteremia (9). The only one of these three types that causes fatalities is the acute hematic phase, known as acute Bartonellosis, or the acute phase of Carrion’s disease (Oroya Fever). It is characterized by the following symptoms after a 7-to-210-day incubation period: fever, severe anemia, cephalalgia, musculoskeletal pain, icterus, asthenia, hepatoemegalosplenia, and altered mental states related to a transient immunosuppressive condition. At this point, patients are bacteremic and erythrocytes may become 100% parasitized.

For centuries, this alkaloid, produced from the bark of an Andean tree, provoked international disputes and intrigue as scientists pursued the cure for malaria.
Next time you catch an inexplicable chill on a hot day in the jungle and an old country doctor prescribes "powders of the countess" ground from "Jesuit's bark" peeled off a "fever tree," you probably have a touch of malaria, and what you will be taking is quinine, which for centuries in one form or another has been the best treatment.
Quinine is the strongest of four fever-fighting alkaloids found in the bark of trees of the genus Cinchona, belonging to the Rubiaceae family, of which there are some twenty-three species, all native to South America, growing along the eastern spine of the Andes. While quinine was not isolated in pharmacological form until 1820, cinchona bark had been known by Europeans since the early seventeen century as a cure for malaria in all its various forms, known then by such archaic names as the ague, tertiana, quartana, and double quartana fevers.
It is a peculiarity of how scientific knowledge often moves onward—in periodic lags and spurts, in alternating breakthroughs and dead ends—that, for more than one hundred years, cinchona was the well-known bark of an unknown tree that, for more than two hundred years, cured the well-known symptoms of an unknown disease.

La próxima vez que en un día caluroso, de selva, sienta escalofríos sin entender por qué y un anciano médico rural le recete los “polvos de la condesa” los cuales se obtienen al moler la “corteza de los Jesuitas” tomándola del “árbol de la fiebre”, es porque probablemente usted haya contraído malaria y esté tomando quinina, la que de una forma u otra ha sido el mejor tratamiento durante siglos.
La quinina es la más fuerte de cuatro alcaloides antifebriles y que fueron encontrados en la corteza de árboles del género de la Cinchona perteneciente a la familia de los Rubiaceae, de la cual veintitrés especies, todas originarias de América del Sur, crecen a lo largo de la Cordillera de los Andes. Mientras a la quinina no se le aislaría en su forma farmacológica sino hasta 1820, los europeos ya conocían la corteza de la cinchona desde comienzos del siglo XVII como una cura contra la malaria en todas sus variadas formas, conocida hasta en ese entonces por nombres primitivos como la fiebre palúdica, la terciana, la cuartana y doble quartana.
Es algo muy singular cómo los conocimientos científicos generalmente están bien encaminados – en medio de constantes atrasos, avances cada cierto tiempo y puntos muertos – al punto que, por más de cien años, la cinchona fue la reconocida corteza de un árbol desconocido el cual alivió, por más de doscientos años, los conocidos síntomas de un desconocido mal.
