Marilyn Amouyal
: the Lichtenstein technique definitely uses a mesh. "fer" is sometimes used to refer to a stapler that can be used during hernia repair, but Lichtenstein does not seem to be associated with a particular stapler. See www.globalfamilydoctor.com/ mejfm-journa

6 hrs
type of suture material
The text asks for the length of the suture and whether the "fer" perhaps suture material needed to be cut again. I do not feel that fer refers to the mesh. The hernia repairs I have seen the surgeon cut the mesh, placed and sutured it in place. Fer in this context could refer to the material used for suturing. I am not 100% certain.

diathermy
another stab in the "pénombre"
The only "fer" I can think of in the surgical context is "fer à cautériser" - diathermy instrument. This would fit in with "utilisation du fer", but "type Lichtenstein" and "re-découpage du fer" ???
This must be either closed-shop surgeon-speak or a typo!
