: A LA CARTE with or without accent (as a native I prefer with) is just PERFECT! Although I like "mix & match" from Melissa I think A LA CARTE here is the best answer here.

DIY & mix and match are just English-language versions of the "a la carte" idea.
I can se your thinking behind "per item", the problem I see though is that it's not a list of per-item prices, as most of the items seem to be sold in quantity - this makes me wonder whether you could call this a "takeaway" or "picnic" menu?

: agree for mix and match, quite trendy, used a lot everywhere in the UK now. Disagree with DIY as, although not incorrect, it is so much more used for home improvements etc.
