You are right, Amel. thank you. I felt the same way as I looked at it again. These are words written by the patient or "spoken" by the patient. Therefore statements would be more appropriate, in agreement with what you just suggested. Thank you!

These are called statements and not sentences or phrases in this context. The reason is because the patient is asserting or affirming information about his or her condition.

They are not merely "sentences" (which you might find on a language exam, for example).
