As your text mentions "切除例" 原発巣術 seems to be a short for "原発巣摘出術", so it can be translated "primary site extractio". However, preference of MedDRA term for extractio is "removal". It depends of what target your reader is. Pharmaceutical comapny preger to use MadDRA tems.

primary focus

"原発巣術" is not correctly phrased. It should be "原発巣" plus "術後". "primary focus" is the term for "原発巣".

operation on primary lesion [or primary growth]

A tumor has not clear borders. It is like something spreading. So,probably that the reason for the word 巣.
