: "گزشتہ طبی روداد" conveys the meaning and easy to understand for common people. "سرگزشت" is also common for "History" but "گزشتہ روداد" is easier for masses. It is my assumption.

Thank Qudsia. I also feel it makes more sense to say: اپنے سابقہ طبی کوائف ہمراہ لائیں as opposed to اپنی سابقہ طبی تاریخ/سرگزشت ہمراہ لائیں.
