1 PATIENT COUNSELING
Case should be registered with the mutation report to discuss if additional genetic testing of the couple or other family members is required
2 PATIENT REGISTRATION
Blood samples of related family members collected by PGT laboratory and informativity test is performed in order to understand which copy of the gene contains the disease causing mutation in each couple
3 PGT SETUP REPORT
IVF procedure should not be initiated before “Preliminary Setup Report” is generated by PGT Laboratory. Embryo biopsy on each embryo is performed by Embryologist
4 PGT-M
PGT-M is performed and results are shared with the IVF center
5 PGT-A
PGT-A is performed on the unaffected embryos selected with PGT-M

With the spread of trophectoderm biopsy and vitrification methods together with the development of whole genome amplification technologies, both single gene disease testing in embryos and euploid embryo selection with 24 chromosome screening became possible. Eventhough single gene disease patients are relatively fertile and young, spotaneous abortion rate is still high in these cases. When embryos are included in 24 chromosome aneuploidy test in addition to single gene disease screening, it was reported that pregnancy rate increases from 45 % to 68% and spontaneous abortion rate decreases from 15 % to 5 %.

• У разі термінової потреби у народженні HLA-сумісного брата/сестри
• У випадку невдалої імплантації ембріону або викидня після дослідження ПГТ-М

Risk of injury to the patient
There is a risk of electric shock if the following conditions apply simultaneously:
The telescope is inserted in the patient.
The light-guide cable is connected to the telescope but not to the light source.
The light source connector of the light-guide cable comes in contact with a conductive surface.
Completely connect the endoscope before inserting the endoscope into the patient.
CAUTION
Risk of injury to the patient
Light sources emit large amounts of energy.
As a result the connectors of the endoscopic equipment and the distal end of the telescope become hot.
There is a risk of:
thermal injury to the patient’s tissue (e.g., from prolonged exposure to the intense illumination in cavities with small lumens, or if the distal end of the endoscope is placed into close proximity to the tissue).
burns to the patient’s or user’s skin.
burns or thermal damage to surgical equipment (e.g., surgical drapes, plastic materials).
Do not place the endoscopic equipment on the patient’s skin, on flammable materials, or on heat-sensitive materials.
Set the output power of the light source to the minimum level that is necessary for a sufficient illumination of the target area.
Avoid prolonged exposure to intense illumination.
Switch off the light source or set the light source to standby mode whenever the light source is not in use.

Source text - Russian Анамнез: в ноябре 2017г появились периодические боли в правой половине живота, диарея, тошнота, по месту жительства проведена симптоматическая терапия, без эффекта. В июле 2018г. усиление болей, слизь с кале, запоры по 3 дня., обследовалась по месту жительства.
Перитонеальных симптомов нет. Опухолевые образования через переднюю брюшную стенку не пальпируются. По данным КТ ОБП+ОГК от 14.01.2018 в легких очаговых и инфильтративных изменений не выявлено. Просветы бронхов не изменены. Лимфатические узлы в средостении, в корнях легких и в подмышечных областях не увеличены.
Гистологическое заключение: от 11.02.2019; Макроописание: В едином блоке отрезок подвздошной кишки длиной 16 см со слепой кишкой и восходящей ободочной кишкой длиной 18 см, с червеобразным отростком длиной 7см, аппендикс обычного вида. На разрезе на слизистой толстой кишки в области баугиниевой заслонки на расстоянии 18 см от проксимального и 19,5 см от дистального краев резекции определяется изъязвленная опухоль блюдцеобразной формы, представлена разрастанием белесой ткани гомогенного вида, с участками некроза, размерами 3,1x1,9x3,2см.
Лапаротомия срединным доступом. При ревизии с использование ИОУЗИ во втором и третьем сегменте печени определяются 11 опухолевых узлов ранее не визуализируемых на КТ до 1см в диаметре, в С4 определяются 8 опухолевых узлов до 1 см в диаметре ранее не визуализируемых на КТ.
Учитывая интраоперационные данные, расходящиеся с данными предоперационного инструментального обследования решено ограничиться выполнением паллиативной правосторонней гемиколэктомией.
У больной рак толстой кишки, мтс в печени. 31.01.19 выполнена попытка хирургического удаления очагов в печени, однако при ревизии состояние признано неоперабельным, выполнена паллиативная правосторонняя гемиколэктомия. Проводилась инфузионная, антибактериальная, антисекреторная терапия. Гистологически: в стенке кишки разрастание низкодифференцированной аденокарциномы. Рекомендована консультация химиотерапевта.
Translation - English History of the present illness: in November 2017 there were occasional pains in the right half of the abdomen, diarrhea, nausea. Symptomatic therapy was carried out at the place of residence without effect. In July 2018 pain increased, mucus in the feces was detected, patient was constipated for 3 days. Medical examination was carried out at the place of residence.
No peritoneal disorders symptoms. Tumor formations are not palpable through the anterior abdominal wall. According to abdominal and thoracic CT scan of 14 January 2018, there are no focal and infiltrative changes in the lungs. The lymph nodes in the mediastinum, in the roots of the lungs and in the axillary regions are not enlarged.
Histopathology Report of 11 February 2019.
Gross description: A segment of the ileum 16 cm in length with the caecum and an ascending colon 18 cm in length, with a vermiform appendix 7 cm in length, a normal appendix are in a single block.
On an incision of the mucous membrane of the colon in the area of valve of colon at a distance of 18 cm from the proximal and 19.5 cm from the distal edges of the resection, an ulcerated saucer-shaped tumor, represented by an overgrowth of whitish homogeneous tissue with areas of necrosis and measuring 3.1x1.9x3.2 cm is determined.
Laparotomy with a median approach. During revision with the use of ioUS 11 tumor nodes up to 1 cm in diameter, which were not previously visualized on CT, were identified in the second and third segments of the liver, 8 tumor nodes up to 1 cm in diameter, that were not previously visualized on CT, were determined in SVI.
Considering the intraoperative data, which differ from the data of the preoperative instrumental examination, it was decided to limit the therapy to palliative right-sided hemicolectomy.
A patient has colon cancer, liver metastases.
On January 31, 2019, an attempt of surgically remove lesions in the liver was made. However, upon revision, the patient's condition was considered inoperable, and palliative right-sided hemicolectomy had been performed.
Infusion, antibacterial therapy and antisecretory treatment were carried out.
Histological examination revealed overgrowth of poorly differentiated adenocarcinoma in the intestinal wall. Consultation with a chemotherapist was recommended.
