dichiarazione di inizio (della procedura) di scioglimento
Scusa, non ho riferimenti, ma io la tradurrei così. Statement di solito è una dichiarazione. Se lo scioglimento è volontario, la società deve solo dichiarare che si dissolve. Se poi ha dei debiti, è un'altra questione.

The asker said in another question that it is a US certificate of existence. I found this online which seems to be a similar text (judging from his/her questions).

My point is that if the company had already been declared bankrupt, the Secretary of State wouldn\'t be allowed to issue this certificate - in my opinion - but it may refer to the fact that the company has given notice that such a procedure has started.
But I agree with you and Danilo that it has to do with dissolution due to fallimento.
After sleeping on it and reading De Franchis long definition, I would actually use this simple common expression:
\"(non) è in liquidazione\"

: Ma come fanno a fargli un certificate of existence dopo che è stata dichiarata fallita? Il certificato dice di solito che la soc. is authorized to transact business.

I don't see where you get the cert. of existence from. In the UK, a trading cert is issued. But I'm unsure this is even GB where 'a copy of the winding-up order' would be more usual.
