In my computer days we had a Change Control System used to record and control all the changes made to software and hardware. In this case, the report is listing all the recorded (LOGGED) changes made.

Really I am not sure of it since the listing shows the changes made and there is no indication of any revision or version data. Only when and what/Who is
shown....
