Source text - English 2. Basic concept of hospital administration
1) Significance of hospital administration
Today, due to the rapidly increasing phenomenon on the demand of medical treatment, the scale of hospital organization is being expanded and specialized. So, then by the heavy workload, not only the ordinary medical, but also the educational, research, religious, and various social service role have become the responsibility of hospital. Therefore, in order to conduct all these functions, more systematic administration is needed, covering all the way from setting, planning, and establishing the hospital operation objective to link-up administration which connects the planning affairs.
These serial significance represents of the tendency of hospital in which the value of hospital administrator has been emerged so far these days, it’s been finally moved
from the dominance of physicians from finally to the ones of administrative manager.
2) Governance of hospital administration
In order to effectively accomplish the multifarious purposes of hospital organization, there must be accelerating operational management which leads and facilitates this, and here includes rationalization process of objective activities such as goal setting, policy making, planning, organization, and regulation as a transitional basis of psychological, and social management on human behavior including authority, decision making, communication, and indexicability as dynamic foundation of administration. This is used as the meaning itself of managerial administration in a narrow sense.
There must be the management by each element as supplementary/continuous function on lead-accelerating function, and this, in detail, speaks of personal management on people, financial management on the finance, facility management on facility, and the management on environment.
Earlier, we went through the system of hospital management which is pivoted in functional structure off hospital administration, but here we would focus on hierarchical structure of it.
Originally, the differentiation on hierarchical function is drawn from the formational process of organization, and it can be classified as administrative and working function. When we say working function, it is the function which is directly and realistically in charge of task to realize the objective of hospital in a more concrete way. And for this, hierarchial function is the planning, accelerating, and controlling activities on the task to be conducted in order to clarify the purpose and meaning of working function, and making it more valuable and efficient.
In short, entailing the expansion and complexification of today’s hospital function and on the operation of mass-scale hospital organization, how to harmoniously form these functions is the significant task given. Therefore, as the hospital organization keeps diversified and especially due to the differentiation of those functions by class, now the class of management is divided into three; top management executive, middle supervisory management zone, and lower supervisory management zone.
(1) Top management executive
Top management executive is the class conducting overall management of hospital as administrative class which is located at the highest level in the hospital administration. In other words, it sets overall objective of hospital, and on that, establishes the basic guidelines and overall plans, and to conduct these overall plans, it sets managemental organization, and finally conducts overall control. In this top management executive, the head of Board of directors, director of hospital, and division of administration, medical affairs, and nursing are included.
For the hospital organization, though the top manager is the director of that hospital, but when we speak of the top management executive, not only the director but also the supporting medical superintend and other officials are included as well.
(2) Middle supervisory management zone
The hospital organization forms the part unit according to the vertical differentiation by the purpose and horizontal differentiation by technical elements of means. The middle supervisory management zone speaks of the administrative person in charge who is responsible for the management on these. In other words, there is only one middle supervisory management zone in the small-scale hospital, but the comparatively large-scale hospital forms several classes such as superintend and section chief.
On the fact that the top management executive performs of the overall management of hospital, the middle supervisory management zone materializes the plans from the top management executive in each section, and performs of the comprehensive management on that part. The major function of these is to clarify the directives, make each members be well-known, and disseminate the order and direction to lower supervisory management zone in order to conduct each part’s purposes. Not only these, they establish the task plans on assigned area, maintains smooth coordinated relations with other parts to pursue execution, and leads the lower supervisory management zone, which is the front-line supervisory zone.
(3) Lower supervisory management zone
When we say lower supervisory management zone, it is the group of managers which is generally called as front-line supervisory zone, and they are in the class who are responsible for the task management function. In other words, they are the ones entitled with the title, subsection chief, chief, or head nurse, and they directly contact with other classes, guide their task, and accelerate it.
Therefore, they perform their mission of facilitating and leading the process of tasks in order to leverage the outcome and correct the progress agenda just according to the decided course of action by the executive manager. They prepare the initiative of works in detail, accelerate the execution of work, confirm the completion of it, and facilitate the improvement of them as well.
So, the structure of each three classes under hospital administration can improve the efficiency of hospital management by conducting unilateral activities under close coordinated system each other.

१) अस्पताल प्रशासनको महत्त्व
वर्तमानमा मेडिकल उपचारको माग द्रुतगतिमा बढीरहेको हुनाले अस्पतालको सङ्गठन पनि व्यापक तथा विशिष्टीकृत हुँदै गएको छ। यसको कार्य यति धेरै बढेको छ कि आज अस्पतालले मेडिकल सेवा मात्र प्रदान गरेर पुग्दैन। शैक्षिक, अनुसन्धान सम्बन्धी, धार्मिक तथा अन्य विभिन्न सामाजिक भूमिकाहरू पनि अस्पतालका उत्तरदायित्व बन्न पुगेका छन्। यसकारण यी सबै कार्यहरूलाई सफलतापूर्वक सम्पन्न गर्नका लागि अस्पताल सञ्चालनका उद्देश्यहरू तय गर्ने, योजना गर्ने र स्थापित गर्ने देखि योजनाका मामिलासँग जोड्ने प्रशासनसित सम्बन्ध निर्माण गर्ने कार्यसम्मका लागि व्यवस्थित प्रशासनको आवश्यकता पर्दछ।
अस्पताल सञ्चालनमा अहिलेको समयमा अस्पताल प्रशासकको भूमिका कसरी विकास भएको छ भन्ने कुरा यस प्रकारको क्रमगत महत्त्वले दर्शाउँछ । आजको समयममा चिकित्सकको नियन्त्रणबाट बाहिर निस्केर प्रशासनिक प्रबन्धकको क्षेत्रमा अस्पताल प्रशासनको व्यवस्थापन पुगेको छ।
२) अस्पताल प्रशासनको सञ्चालन
अस्पताल सङ्गठनका बहुपक्षीय उद्देश्यहरू प्रभावकारी रूपमा पूरा गर्नका लागि तीब्र गतिमा अगाडि लैजाने खालको सञ्चालन व्यवस्थापन हुनु पर्दछ। यस प्रकारको व्यवस्थापनले लक्ष्य तय गर्ने, नीति निर्माण गर्ने, योजना गर्ने, सङ्गठन गर्ने तथा अधिकार, निर्णय लिने, सञ्चार गर्ने र सूचिकरण गर्ने नै प्रशासनका गतिशील आधारका रूपमा मानवीय व्यवहारको अन्तरिम आधारमा मनोवैज्ञानिक तथा सामाजिक व्यवस्थापनलाई नियमन गर्ने जस्ता वस्तुगत क्रियाकलापको वैज्ञानिकीकरणको प्रक्रियालाई नेतृत्व प्रदान गर्दछ र सहजीकरण गर्दछ। यसैलाई संकुचित अर्थमा व्यवस्थापकीय प्रशासनको रूपमा लिइन्छ।
नेतृत्वलाई बढवा दिने कार्यलाई हरेक तत्त्वबाट सहयोगी तथा निरन्तरता दिने व्यवस्थापन हुनुपर्दछ र विस्तृत रूपमा यसले मानवहरू, आर्थिक पक्षहरू, सेवाहरू तथा वातावरणको व्यवस्थापन गर्ने विषयमा जनाउँदछ।
अस्पताल प्रशासनको कार्यगत संरचना बाहिर प्रमुख भूमिकामा रहने किसिमको अस्पताल व्यवस्थापन विधि पहिले पहिले हामीले देखि सकेकाछौँ तर यहाँ हामी यसको श्रेणीगत संरचनामा केन्द्रित हुनेछौँ।
प्रारम्भमा श्रेणीगत कार्यको विभेदता सङ्गठनको निर्माण गर्ने प्रक्रियाबाट नै लिइन्छ र यसलाई प्रशासनिक र क्रियाशील कार्य भनी वर्गीकरण गर्न सकिन्छ। क्रियाशील कार्य भन्नाले अस्पतालको उद्देश्यलाई भैतिक रूपमा प्रकट गराउन प्रत्यक्ष तथा वास्तविक ढङ्गले सक्रिय भूमिकामा रहने कार्यलाई जनाउँदछ। यसका लागि क्रियाशील कार्यको उद्देश्य तथा अर्थ स्पष्ट पार्न र यसलाई अझै बढी मूल्यवान तथा प्रभावकारी बनाउनका लागि गरिने कार्यहरूको योजना गर्नु, तिनीहरूलाई छिटो सञ्चालन गर्नु र र नियन्त्रण गर्नु भनेको श्रेणीगत कार्य हो।
छोटकरीमा भन्नु पर्दा आजको अस्पतालको कार्यको विस्तृतीकरण र जटिलीकरणका परिणामलाई उल्लेख गर्दा र वृहत स्तरका अस्पताल सञ्चालनमा ल्याउँदा महत्त्वपूर्ण कार्य भनेको यी कार्यहरू बीच कसरी सन्तुलन वा तालमेल निर्माण गर्ने भन्ने नै हो। यसर्थ अस्पताल सङ्गठन विवधीकरण हुँदै जाँदा र विशेष गरेर यी वर्गीकरणका कार्यहरूको फरकका कारणले व्यवस्थापनलाई तीन समूहमा विभाजन गरिएको छः व्यवस्थापनको उच्च कार्यकारी तह, व्यवस्थापनको मध्यम सुपरीवेक्षण तह र व्यवस्थापनको तल्लो सुपरीवेक्षण तह।
(१) व्यवस्थापनको उच्च कार्यकारी तह
व्यवस्थापनको उच्च कार्यकारी तह भनेको अस्पताल प्रशासनको सबैभन्दा माथिल्लो तहमा अवस्थित सम्पूर्ण अस्पताललाई एउटा प्रशासनिक एकाइको रूपमा व्यवस्थापन गर्ने समूह हो। अर्को शब्दमा भन्नु पर्दा यसले अस्पतालको सम्पूर्ण उद्देश्य तयार गर्दछ र त्यसकै आधारमा आधारभूत निर्देशनहरू तयार गर्दछ। त्यसपछि यसले यी सम्पूर्ण योजनाहरू कार्यान्वयन गरेर व्यवस्थापकीय सङ्गठनको प्रारूप तयार पार्दछ। अन्ततः यसले सम्पूर्ण सङ्गठनलाई नै नियन्त्रण गर्दछ। व्यवस्थापनको यस उच्च कार्यकारी तहमा निर्देशकहरूको समितिका प्रमुख, अस्पतालका निर्देशक, प्रशासन विभागका प्रमुख, मेडिकल मामलाका प्रमुख तथा नर्सिङ्गका प्रमुख संलग्न हुन्छन्।
अस्पताल सङ्गठनका लागि उच्च प्रबन्धक सो अस्पतालको निर्देशक भए तापनि हामीले अस्पताल व्यवस्थापनको उच्च कार्यकारी तह भन्नाले उक्त निर्देशकलाई मात्र नभई मेडिकल सुपरीवेक्षक तथा अन्य सहयोगी मेडिकल अधिकृतहरूलाई पनि बुझ्नुपर्दछ।
(२) व्यवस्थापनको मध्यम सुपरीवेक्षण तह
उद्देश्यपूर्वक सिधा लम्बवत भिन्नता र साधनहरूको प्राविधिक तत्त्वका लागि तेर्सोरूपमा प्रस्तुत हुने विभिन्ता अनुरूप अस्पतालको सङ्गठनले हरेक खण्डका एकाइ निर्माण गर्दछ। व्यवस्थापनको मध्यम सुपरीवेक्षणको तहले प्रशासनिक जिम्मेवारी लिएर बसेको व्यक्तिसित यी कुराहरूको व्यवस्थापनका बारेमा छलफल गर्दछ। अर्को शब्दमा भन्नु पर्दा सानास्तरका अस्पतालहरूमा एक मात्र व्यवस्थापनको मध्यम सुपरीवेक्षण तह हुन्छ तर तुलनात्मक रूपले ठूला स्तरका अस्पतालहरूले विभिन्न यस प्रकारका समूहहरू सुपरीवेक्षक तथा शाखा प्रमुखका रूपमा सृजना गर्ने गर्दछन्।
अस्पतालको सम्पूर्ण व्यवस्थापनको कार्य व्यवस्थापनको उच्च कार्यकारी तहले गरेजस्तै मध्यम सुपरीवेक्षण तहले उच्च कार्यकारी तहबाट प्राप्त योजनाहरूलाई हरेक खण्डमा साकार गर्दछ र आफ्नो तर्फबाट विस्तृत व्यवस्थापकीय कार्यसम्पादन गर्दछ। हरेक खण्डको आफ्नो भागको कार्य सुचारू रूपमा सञ्चालन गर्नका लागि यिनीहरूको प्रमुख कार्य भनेको निर्देशनहरूलाई स्पष्ट पार्नु, हरेक सदस्यलाई चिनाउनु तथा आदेश र निर्देशनलाई व्यवस्थापनको तल्लो सुपरीवेक्षणको तहसम्म वितरण गर्नु हो। यतिमा मात्र सीमित नरहेर यिनीहरूले तोकिएका क्षेत्रमा कार्ययोजना स्थापित गर्दछन्, यसलाई लागू गर्नका लागि अन्य अङ्गहरूसित राम्रोसँग संयोजन गरिएको सम्बन्ध कायम गर्दछन् र सबैभन्दा अगाडि सुपरीवेक्षण तहको रूपमा देखा पर्ने व्यवस्थापनको तल्लो सुपरीवेक्षण तहलाई नेतृत्व प्रदान गर्दछन्।
(३) व्यवस्थापनको तल्लो सुपरीवेक्षण तह
व्यवस्थापनको तल्लो सुपरीवेक्षण तह भन्नाले साधारणतः सुपरीवेक्षणको सबैभन्दा अगाडिको तहमा रहेर काम गर्ने प्रबन्धकहरूको समूहलाई बुझिन्छ। यिनीहरू कामको व्यवस्थापन गर्ने कार्यप्रति जिम्मेवार हुने वर्गमा पर्दछन्। यिनीहरूलाई उपशाखा प्रमुख, प्रमुख अथवा प्रमुख नर्स जस्ता पदहरू दिइएको हुन्छ र यिनीहरूले अन्य वर्गसित प्रत्यक्ष सम्पर्क राखेका हुन्छन्। यिनीहरूले तिनीहरूलाई निर्देशन दिन्छन् र कामलाई गति प्रदान गर्दछन्।

1. A doctor from the study will examine you and will take some measurements of the size of your ears, eyes, upper lip, and the distance around your head. You will have two digital photos taken of your face. One of these is a 3-D photo taken with a special camera. We will give you a copy of the 3-D photo to keep. This will all take about 30 minutes.
2. Sometime in the next 6 months you will be tested. The tests measure different kinds of learning and memory and are made to be fun to do. You may be asked to answer questions, look at pictures, and do puzzles. The testing session will take about 90 minutes to finish. When you’re done with the testing session you will receive $20.00 gift card.
