IN AND FOR THE COUNTY OF MARICOPA
UNIFORM INTERROGATORIES
1. State your name and address or principal place of business, date of birth, and social security number.
2. Have you been convicted of a felony? If so, state:
a. The original charge made against you.
b. The charge of which you were convicted.
c. Did you plead guilty of the charge or were you convicted after trial?
d. The court and cause number.
3. Have you ever been a party to a civil lawsuit? No. If so state:
a. Were you plaintiff or defendant?
c. What was the nature of the plaintiff’s claim?
c. When, where and in what court was the action commenced?
d. State the names of all the parties other than yourself.
4. State exactly and in detail plaintiff’s version of how this accident occurred.
5. State specifically and in detail the facts upon which plaintiff’s contention is based that the accident was caused by any negligent conduct on the part of the defendant.
6. Was an investigation conducted concerning the accident in question? If so state:
a. The name, address and occupation of the person or organization conducting the investigation.
b. The date or dates for which the investigation was conducted.
c. Whether plaintiff or anyone acting on plaintiff’s behalf has interviewed or spoken with defendant or any of tits agents or employees about the event in question.

17. Do you have liability insurance or are you aware of any other form of indemnity which you claim is applicable to this accident? If the answer is yes, state:
a. The name of the company or companies, including any excess or umbrella carriers, which you claim provide coverage.

19. State whether you, or anyone else was involved in the accident, ingested any drugs within 48 hours prior to the accident or drank any intoxicating beverages of any kind within 12 hours prior to the accident or to the occurrence of the accident alleged in the complaint.
20. Do there exist any liens, including AHCCCS, Medicare, or any liens provided for by A.R.S. 33-931 et seq., on any recovery you may have or may obtain in this matter? If so, give the amount and entity holding such lien and the nature of said lien.
21. If the accident that is the subject of plaintiff’s claim was an automobile accident, please state the following:
a. Did the vehicle which you were occupying at the time of the accident contain operational seatbelt?

Although I was born in California our family moved to Peru, South America two years later, where we lived another two years. We then moved to Arizona where I grew up in a town full of Spanish speaking people, including both of my parents, my Father was born in Mexic and grew up there and my mother learned Spanish as an adult.
Subsequently I took Spanish in High School, served a 2 year mission in Central America, Honduras, Costa Rica, Panama, and Nicaragua. Upon my return to the State I took a minor in my university studies in Latin American Studies, with a major in Political Science. I graduated and then proceeded to graduate from Law School, and worked as an attorney for a number of years. I made a conscious effort to work with the Latin community.
I then met my wife in Colombia, South America while working and we now have 4 children and 5 grandchildren. I keep my "feet" in the Latin/Spanish world on a daily basis with my wife, her friends, and my inlaws, and with clients. I translate legal documents and write and translate poetry [a hobby].
For enjoyment I love to travel, read, listen to music, play the violin, piano, and guitar, paint, and do things with my wife and our last son, who is 14 and who plays the guitar better than I do.
