Source text - English 17.2 Adaptation to Breast Cancer: A Phase Approach
Adaptation to cancer, a major stressor for anyone, varies widely from individual to individual and is influenced by a variety of factors. Among these are disease stage and where the person is in the disease trajectory [8].
17.2.1 Diagnosis of Cancer: A Crisis Event
Why me? What did I do wrong? Did I cause the cancer? Did someone wish this on me – who was it? Is this punishment? Am I going to die? These are some of the common questions a woman may ask. Fear, anxiety, depression, denial, psychic numbness, confusion, confronting one’s mortality, terror, anger, and loss of trust in God, in self, in one’s body, and in others are common feelings and reactions that women may exhibit [9].
A diagnosis of cancer introduces many alterations in a person’s life that require adjustment, effective problem-solving, and active coping. Integrating the reality of the diagnosis into one’s personal life, tolerating emotional turmoil and stress, accepting help and some dependency, adjusting to the milieu of the healthcare system, planning regular daily routines to undergo treatment, making decisions about treatment options, communicating about the illness/diagnosis and its implications to family and friends (specifically for younger patients, their children, and/or parents), and searching for meaning in the adverse episode they are living are some of the coping tasks identified in the early phase of dealing with diagnosis. All of these must be juggled while the woman simultaneously focuses on trying to get the best care possible or available with the least disruption to life [9].
In looking back, the early phase of care is reported by many women to be the most stressful of their breast cancer experience, that is, living with the uncertainty and trying to come to grips with a new reality while also making the “right” decisions about what to do next. As information about the recommended course of care emerges, and a plan of treatment is determined and therapy actually begins, anxiety generally tends to diminish.

Адаптация к раку, который является главным стрессором для каждого человека, протекает по-разному и зависит от множества факторов.
Среди них – стадия заболевания и то, где человек находится на траектории болезни [8].
17.2.1 Диагноз «рак»: переломное событие
Почему именно я? Что я сделала не так? Это я спровоцировала рак? Неужели кто-то пожелал мне этого – кто это был? Это наказание? Неужели я умру? Вот некоторые из наиболее частых вопросов, которые может задавать женщина. Страх, беспокойство, депрессия, отрицание, психическое оцепенение, замешательство, осознание своей смертности, ужас, гнев и потеря веры в Бога, в себя, в свое тело и в других – это обычные чувства и реакции, которые женщина может испытывать [9].
Диагноз «рак» вносит много изменений в жизнь человека и требует адаптации, эффективного решения проблем и активной борьбы. Включение диагноза в свою личную жизнь, терпимость к эмоциональным потрясениям и стрессу, принятие помощи и некоторой зависимости, адаптация к медицинской среде, планирование привычных каждодневных процедур с учетом лечения, принятие решений о варианте лечения, сообщение о болезни/диагнозе и последствиях членам семьи и друзьям (особенно для молодых пациенток, их детей и/или родителей), а также поиск смысла в этот тяжелый период – это лишь некоторые из задач, которые предстоит решить на начальном этапе процесса принятия диагноза. Женщине приходится искать решения, сосредоточив в то же время свое внимание на попытке получить наилучший уход, возможный или доступный с наименьшим влиянием на жизнь [9].
По прошествии времени многие женщины рассказывают, что начальный этап лечения РМЖ является самым трудным, то есть они живут с неопределенностью и пытаются справиться с новой реальностью, одновременно принимая «правильные» решения о том, что делать дальше. По мере появления информации о рекомендуемом курсе лечения, составления плана лечения и непосредственного начала терапии тревога обычно начинает уменьшаться.
