Jedná sa o výpis z listu vlastníctva. LV má svoje číslo a potom v rámci popisu sa uvádza číslo EL (čo je číslo evidenčného listu).

Certificate of Title/Transaction Record

Explanation:
Certificate of Title is the term used in Australia for LV. There is no such thing as EL here - these are Transaction records which are now kept electronically. I guess in your context LV is Certificate of Title (who owns the property) and EL is the Transaction Record issued every time any transaction is completed on the property.
I would use "Transaction Record" - and then "Transaction Record Number" for "Cislo EL".

Last Note: Target culture is important as this will vary from country to country, so you should specify to which country it relates.

Following the asker\'s note just one comment: \"File Number\" instead of \"Transaction Record Number\" would be just fine - the transaction record is still a file. It does sound a bit more generic, but that might be a good thing...

For LC, Eviden�n� list nie je TITLE DEED, preto�e s�m o sebe nezaklad� n�rok na vlastn�ctvo nehnute�nosti. Je to len dokument, v ktorom sa uv�dzaj� presn� inform�cie o majite�ovi a o nehnute�nosti.

Thank you for your help folks. Although I closed this question without grading, the answers bellow will be a good point of reference for Slovak translators searching for real estate terminology.
And, for the record, evidencny list is the document kept on file at the Slovak Land Registry (cadastre) that contains all pertinent information about the owner and a very detailed description of the property.

Well aware of the differences in land registration systems (title-based vs. deed-based), I did not intend to start a discussion about list vlastnictva, my question is strictly related to evidencny list.

1/yes, list vlastnictva is deed in Canada as well
2/we are talking about "evidencny list", this "evidencny list" indicates that the property is in evidence but it is not the same as "list vlastnictva" - deed. They are not the same.

Assessor's office computes real estate taxes. They have record of everything, because it is source of income for Counties in US.

List vlastnictva would be in this case deed, in order for US owner to understand it. All the other stuff will not make sense to him. In US each real estate lot has number and under that number it is registered in County assessor's office.

list vlastnictva ->is a documet showing who owns the property; registracny list/dokumet -> the property was registered; and evidencny list -> the property is in evidence and there must be an evidence number or File Number.

The translation is for a US client. And, to be honest, the eviden�n� list has me still somewhat puzzled. Most importantly, I am not sure if it refers to the owner or the transaction because it is called "eviden�n� list vlastn�ka". So far, I've been considering FILE NO. Does anyone know more about it?

If this is a document, showing who owns the property, and if it is for US, none of translations will make sense. Look at terms deed and title to the property.
