I think that the rupture is limited to the mucous membrane. (I mistakenly hid my answer so I am reposting my suggestion. Please forgive me for my hitting the wrong button.)

You're right, more context would be a great help. I wonder if an MD would know without more context...I also thought that it could mean the opposite, without damaging the mucous membrane.

I suppose it means that the wall is ruptured but not the mucous membrane. It would be useful to say the least to know what the subject of the sentence is.
