To me, two things are clear: the abbreviation is not a very common one to say the least (could be some error somewhere), and the author is most probably talking about the internal diameter of the atrium. If I would have to translate it, I would contact the author or translate it as "internal diameter" and put a comment saying something like ¿correcto? or ¿de acuerdo? Nevertheless, it is vexing.

First of all, please let me apologize for an erroneous reply - I'll delete it at the end of the day, so people have a chance to read this discussion.
In itself, the translation I gave is correct, but in the present context it is wrong. The fact that an English abbreviation is used in a Spanish text is not unusual. In
http://www.elsevier.es/cardio/ctl_servlet?_f=40&ident=32
it says: "Lamas, en 1989, ideó el índice de esfericidad telesistólico y telediastólico (SSI y DSI)".
However, the cited text goes on to say "Este índice es calculado como el volumen del ventrículo izquierdo […] dividido por el volumen de una esfera hipotética […]."
This means that the index refers to the ventricle, is dimensionless, and cannot be measured in mm.
The internal diameter of the right atrium is approx. 4 cm with a range of 2.5-4.9 cm (you'll find these numbers if you Google "diameter of the right atrium is"). This fits with a dilated value of 51 mm.
So I think that they are talking about the internal diameter of the atrium, although I find a "diámetro sistólico interno" a bit strange when talking about atria; I did not find this combination of words (when talking about atria) in Google either.
