This seems to be an overview or statement issued by the landlord to tenants. I can't see much wrong with 'issued'.

it is indeed confusing ;)) and something we have to bear when translating legal documents. Was the original document faxed?

I think this, albeit in strangely-worded Dutch, refers to an overview that is being supplied to the tenant, and they're talking about the period it covers and when it is supplied to the tenant. Now I'M repeating myself!

volgens mij dient het tweede 'verstrekt' weggelaten te worden voor een goede zin
