A stat report is an urgent or accelerated report of the test results, supplied in a shorter time period than would normally be the case:
"If you require the report quickly, you can request a STAT report which will arrive within approximately 2 weeks. There is an additional fee."

On the form to which you have provided a link (and as you say there are other similar ones around), the first two options are "Call stat report" and "Fax stat report". I think "call" means by telephone; elsewhere on the form it says "call me on my cell phone with results". So I read "call" and "fax" as verbs, meaning "send results by phone" and "send results by fax".

Fax INMEDIATO
STAT: Medical term used to imply urgent or rush. It may appear in lower case letters as stat or in capital letters as STAT, as in "Treatment may include STAT surgery." The term is derived from the Latin word "statim" which means immediately.
