It's part of the patient data, between 'esetszam' en 'TAJ', on a form named 'lelet'
It's also on a form named 'vizsgalatkero lap' (for lab tests), and below it is 'minta azonosito'

The only possible explanation I can think about is that "teli" is the end of a word not completely visible in the form. So, IF this is the case, "vérvételi azonosító" can be a possibility.
