Parent survey special high school (for students with disabilities)
This is the American version with "high school students" as opposed to upper secondary and pubils which strike as more British English. I think survey is used more commonly here than questionnaire. Also, "special school" is used in English relatively similarly to särskola in Swedish, although sometimes it might help to specify what it's for, but it's not necessarily necessary.
It is not specified in the Swedish text whether it refers to a special school for students with learning disabilities, or physical disabilities, or what, so unless the context says something in particular, it might be better to just say "disabilities" or to leave out a description, and just say special high school/special upper secondary school (or whatever the British English is exactly).
