It’s not that simple.
After over two decades of this epidemic we have learned a great deal about its transmission; about care options; about its global impact and about what preventive measures are most feasible. The intricate link between prevention and care is one that we are only now beginning to fully grasp and, with this understanding, comes the responsibility to ensure that mainstream prevention and care programs act on the synergies that exist between and among these elements. Yet it sometimes seems as if these lessons are not sufficiently considered under the weight of other political, ideological and even moral considerations. An excellent case in point is the ABCs (Abstinence; Be faithful and Condom usage), which is all too frequently bandied about as some sort of mantra to cure all of the community’s woes.
The bias of the ABCs is based on the assumption that individuals all have an innate and equal power to make perfectly correct decisions about every issue in their sexual and reproductive health lives. Abstain until you are married and, if that’s not an option, be faithful to one committed (and presumably HIV negative) partner. Alternately – and with an underlying tone implying that this choice is only for those who fail to make the grade on these first two options – use a condom with every partner. It all sounds so simple. But this gradation of ‘choices,’ with condom promotion being emphasized as the third viable option, completely ignores and fails to celebrate that we are all – irrespective of health status, gender or class – sexual beings.
This shortsightedness is all the more apparent when looking at the sectors that emphasize ‘abstinence only’ and moral education as their sole prevention initiatives.
Behind the relative simplicity of the ABCs lies a mistaken notion that ignores the power and gender inequalities that have seriously impacted the best planned interventions. Feeling ashamed for failing to prevent infection inherently becomes a deterrent against the infected proactively seeking out the support and services they need. In this way, the epidemic will continue to find a fertile breeding ground inside the communities most critically in need of help.
Experts pondering the Uganda success story recognize that no single factor resulted in the gradual stabilization and decline of the country’s epidemic. It was the unique combination of early political will translated into action, with the broad involvement of players from a wide range of sectors in a number of well-orchestrated interventions. Attempting to place the responsibility securely at the door of abstinence-only and faith-based programs is myopic and dangerous. While these programs undoubtedly had an impact, along with a host of other interventions, it is unwise and irresponsible to attribute portions of effectiveness among specific elements of a comprehensive program initiated in the early nineties.
While the intentions of the ABCs are noble, greater emphasis needs to be placed on how each new HIV infection takes place within its own socio-economic and political dimension. These dimensions – while they exist as theoretical concepts for many of us – have a profound impact on the kinds of messages that are given to those most vulnerable. And sustainable prevention and care initiatives are those that recognize these differences while initiating programs across broader dimensions.
While this epidemic is forcing all sectors of the population to reexamine issues of culture, tradition, power and status, it is clear that the most effective and practical prevention tool we currently have rests in promoting condoms. And condoms – as the final letter in the ABCs – should not be weighted against arguments of abstinence and faithfulness. In doing this we are not only branding those who develop STIs or become HIV positive as failures and worthy of moral indignation, but we are failing to acknowledge that there are constraints in all of our diverse lives that make this specific menu of choice difficult, if not impossible. We should put our global energies behind aiming to ensure that sex is safe and that condoms are readily available. If we can win this strategic battle, we will make one the most important contributions possible in fighting this epidemic: an environment free of judgment and prejudice. In this way, we can make choice a concept that has meaning.

Una estrategia no tan sencilla en la práctica
Luego de más de veinte años, hemos aprendido mucho sobre la epidemia del VIH: las formas de contagio, las opciones de atención, las repercusiones a nivel mundial y qué medidas de prevención son más viables. Recién ahora empezamos a comprender en su totalidad el vínculo complejo que existe entre prevención y atención. De este conocimiento deriva la responsabilidad de asegurar que los principales programas de prevención y atención actúen sobre la sinergia que existe entre estos dos elementos. Sin embargo, a veces parecería que este aprendizaje no basta cuando entran en juego otras consideraciones de índole política, ideológica y también moral. Un excelente ejemplo es la estrategia de prevención “ABC”. Esta sigla corresponde al acrónimo en inglés de “Abstinence, Be faithful and Condom usage”, que en español significa: “abstinencia, ser fiel y usar el condón”. Con mucha frecuencia, se habla de esta estrategia como una especie de mantra que cura todos los males de la humanidad.
La parcialidad de este “ABC” surge de la presunción de que todos los individuos poseen el mismo poder innato para tomar decisiones del todo correctas sobre cualquier asunto relacionado con su vida sexual y su salud reproductiva. Esta estrategia promueve: abstenerse hasta el matrimonio; si esto no es posible, mantenerse fiel a una pareja estable y que sea presumiblemente VIH negativo. De forma alterna, fomenta el uso del condón con todas las parejas, pero con un tono que sugiere que esta elección es sólo para aquellos que no tuvieron éxito con las dos primeras. Todo parece muy sencillo. No obstante, esta escala de “alternativas” en la cual la promoción del condón se subraya por ser la tercera opción viable, ignora por completo y no celebra que todos somos seres sexuados, sin hacer distinciones de género, clase o estado de salud.
Esta falta de visión se hace más evidente cuando se trata de los sectores que promulgan “solamente la abstinencia” y la educación moral como las únicas medidas de prevención. Detrás de la relativa sencillez del “ABC” yace una idea equivocada, la cual ignora las desigualdades de poder y de género que han repercutido seriamente en las intervenciones mejor planeadas. La vergüenza por no poder prevenir el contagio causa en forma natural el desaliento de las personas infectadas, quienes así no piden en forma activa la ayuda y atención que necesitan. De esta manera, la epidemia seguirá encontrando un campo fértil dentro de las comunidades que más urgen asistencia.
Los especialistas que analizan el éxito del caso de Uganda reconocen que no fue un único factor el que contribuyó a disminuir y estabilizar en forma gradual la epidemia en este país. Fue una combinación excepcional de voluntad política temprana transformada en acción, más la amplia participación de actores sociales de una gama de sectores en un conjunto de intervenciones planificadas con cuidado. El intento de volcar toda la responsabilidad sobre los programas que promueven sólo la abstinencia y la fidelidad es ciego y peligroso. Aunque estos programas sin duda tuvieron un efecto, junto con otras intervenciones, resulta poco prudente e irresponsable atribuir gran parte de la eficacia a determinados componentes de un programa integral que se puso en marcha a principios de los años noventa.
A pesar de que las intenciones de los programas “ABC” son nobles, es necesario darle mayor importancia al modo en que se produce cada nuevo contagio del VIH dentro de su propia dimensión política y socio-cultural. Estas dimensiones, que para muchos de nosotros son meros conceptos teóricos, tienen una profunda influencia en los tipos de mensajes que se transmiten a las personas más vulnerables. Una iniciativa de prevención y atención sostenible reconoce estas diferencias al mismo tiempo que pone en ejecución programas a través de dimensiones más amplias.
En tanto que esta epidemia obliga a todos los sectores de la población a volver a analizar cuestiones de cultura, tradición, poder y estatus; es indiscutible que la herramienta de prevención más práctica y eficaz que tenemos actualmente se apoya en la promoción del condón. Esta opción, por ser la última del “ABC”, no debería confrontarse con la defensa de la abstinencia y la fidelidad. Si hacemos esto, no sólo estamos tildando de condenadas y dignas de humillación a aquellas personas que desarrollan enfermedades de trasmisión sexual o se contagian con el VIH, sino que además no reconocemos que existen limitaciones en todas nuestras vidas que hacen que la elección de una alternativa de este menú sea difícil, si no imposible. Deberíamos aunar nuestras energías en procurar garantizar que el sexo sea seguro y que el condón esté al alcance de todos. Si ganamos esta batalla fundamental, lograríamos una de las mayores contribuciones posibles en la lucha contra esta epidemia: una sociedad libre de prejuicios y condenas. Entonces, la palabra “elección” sí tendría sentido.

Source text - English How to report scientific developments on vital issues of the day that are less well understood or in which the connection is less direct is a more complicated question. Global weather patterns are a case in point. Even if we understand some effects of carbon dioxide in the atmosphere, it is difficult to predict the precise chain of events that a marked increase in carbon dioxide will cause.
The distillation of results presented to the public in such cases should reflect at least some of the subtleties of the most current developments. More balanced reporting would of course help. Journalists will seek to offer balance by providing an opposing or competing perspective from another scientist on a given development. But almost all newly discovered results will have some supporters and some naysayers, and only time and more evidence will sort out the true story. This was a real problem in the global warming debate for a while: the story was reported in a way that suggested some scientists believed it was an issue and some didn't, even long after the bulk of the scientific community had recognized the seriousness of the problem.
Translation - Spanish Una cuestión aún más complicada es la manera de dar a conocer los avances científicos sobre temas vitales de la actualidad que son más difíciles de comprender o que presentan nexos menos directos. Un ejemplo puntual es el de los regímenes climáticos del planeta. Aunque comprendamos algunos de los efectos del dióxido de carbono sobre la atmósfera, es difícil predecir la cadena exacta de acontecimientos que provocaría un incremento elevado de dióxido de carbono.
En estos casos, la síntesis de los resultados que se presentan al público debería reflejar por lo menos algunos detalles importantes de los últimos avances. Seguro que una cobertura más equilibrada de los medios ayudaría. Los periodistas deberían procurar mayor objetividad con la presentación de puntos de vista opuestos o alternativos de otro científico sobre el mismo avance. No obstante, casi todos los hallazgos recientes tendrán partidarios y oponentes, y solamente el paso del tiempo y la suma de más pruebas sacarán a la luz la verdadera historia. Durante un período esto fue un verdadero obstáculo en el debate sobre el calentamiento global, ya que el tema fue informado de tal manera que sugería que algunos científicos creían que era un problema y otros no, aún mucho después de que la mayor parte de la comunidad científica había reconocido la seriedad del tema.

Source text - English Such public stirrings are at last getting the attention of politicians and business leaders, who may not always respond to science but have a keen nose for where votes and profits lie. State and local lawmakers have started taking action to curb emissions, and major corporations are doing the same. Wal-Mart has begun installing wind turbines on its stores to generate electricity and is talking about putting solar reflectors over its parking lots. HSBC, the world's second largest bank, has pledged to neutralize its carbon output by investing in wind farms and other green projects. Even President Bush, hardly a favorite of greens, now acknowledges climate change and boasts of the steps he is taking to fight it. Most of those steps, however, involve research and voluntary emissions controls, not exactly the laws with teeth scientists are calling for.
Is it too late to reverse the changes global warming has wrought? That's still not clear. Reducing our emissions output year to year is hard enough. Getting it low enough so that the atmosphere can heal is a multigenerational commitment. "Ecosystems are usually able to maintain themselves," says Terry Chapin, a biologist and professor of ecology at the University of Alaska, Fairbanks. "But eventually they get pushed to the limit of tolerance."
