Source text - English The risk of endometrial hyperplasia and carcinoma is increased when systemic estrogens are administered alone for prolonged periods of time. The endometrial safety of long-term or repeated use of topical vaginal estrogens is uncertain. Therefore, if repeated, treatment should be reviewed at least annually, with special consideration given to any symptoms of endometrial hyperplasia or carcinoma.
If break-through bleeding or spotting appears at any time on therapy, the reason should be investigated, which may include endometrial biopsy to exclude endometrial malignancy.
With vaginal administration, stimulation of the liver by the first-pass effect is avoided and thus, transvaginal estrogens might affect hormone binding proteins and other serum proteins produced by the liver less than oral hormones.
To date, interactions between X and other concomitantly administered drugs have not been reported.
There are no clinical data on exposed pregnancies.
Vaginal lavage should be considered. If accidental ingestion of large quantities of the product occurs, an appropriate method of gastric emptying may be considered.
Translation - Polish Ryzyko rozrostu (proliferacyjnego) oraz raka śluzówki macicy ulega zwiększeniu wówczas, gdy przez dłuższe okresy czasu pacjentce ogólnoustrojowo podawane są estrogeny. Bezpieczeństwo długotrwałego lub wielokrotnego stosowania estrogenów dopochwowo i jego wpływ na endometrium są niejasne. Z tego względu, gdy terapia estrogenowa (antykoncepcja hormonalna) jest powtarzana, pacjentka powinna poddawać się kontroli lekarskiej przynajmniej raz do roku, ze szczególnym uwzględnieniem wszystkich objawów mogących wskazywać na rozrost lub raka śluzówki macicy. Gdy dojdzie do krwawienia międzymiesiączkowego lub plamienia z pochwy w dowolnym momencie terapii, należy zbadać jego przyczynę, np. za pomocą biopsji endometrium celem wykluczenia nowotworowego charakteru zmian śluzówki macicy.
Przy podaniu dopochwowym unika się stymulacji wątroby w ramach efektu pierwszego przejścia leku (z jelita przez ten organ), dzięki czemu estrogeny wnikające do układu krwionośnego przez ścianę pochwy mogą wpływać na białka wiążące hormony i inne białka osoczowe produkowane przez wątrobę w mniejszym stopniu, aniżeli hormony podawane doustnie.
Do tej pory nie odnotowano epizodów wzajemnego oddziaływania pomiędzy preparatem X i innymi lekami podawanymi w politerapii.
Nie istnieją dane kliniczne dotyczące wpływu tej terapii hormonalnej na rozwijający się płód (zarodek).
Należy rozważyć zabieg płukania pochwy. Jeśli dojdzie do przypadkowego połknięcia dużych ilości tego produktu, można rozważyć zastosowanie właściwej metody opróżniania żołądka.
