La regulación de la transcripción de APOE sólo se ha investigado parcialmente en el sistema nervioso central y podría ser extremadamente complejo. La APOE humana existe en tres alelos polimórficos: ε2, ε3 y ε4, lo cual hace necesario recurrir a mecanismos de expresión diferencial de las diferentes isoformas de APOE para entender mejor su rol en la EA, puesto que las variaciones en el locus de los alelos generan cambios en el nivel de expresión de APOE que hasta ahora se han asociado a la EA de manera controversial. El análisis de la regulación transcripcional de estos alelos y las variantes en sus regiones reguladoras no sólo ayudarán a elucidar la etiología de EA, sino también a orientar el desarrollo de fármacos dirigidos para la terapia de EA.
A pesar de que es claro el rol de la vía MAPK/ERK en la EA, también es evidente que los hallazgos en relación con patrones moleculares de la EA pueden variar por cuestiones técnicas en el diseño experimental de las investigaciones y por la severidad de la enfermedad, así que los estudios deberían apuntar a una aproximación global, teniendo como objetivo redes de proteínas amplias y redes de vías de señalización, en lugar de estudiar genes de forma aislada; así, un día se hará posible proponer medidas terapéuticas cuya efectividad se mantenga en el transcurso de la EA.
Los estudios de transcriptoma comúnmente identifican procesos alterados en la EA, ya sea como consecuencia específica de la enfermedad o como resultado de los efectos de la neurodegeneración general; es necesario desarrollar más trabajos que permitan separar los procesos alterados causados por proceso neurodegenerativo, de los que son una consecuencia directa de la EA.
La EA es considerada por la Organización Mundial de la Salud (OMS) como una problemática de salud pública desde el año 2012. A la fecha no hay información actualizada sobre la frecuencia y la carga económica generada por ésta enfermedad en Colombia; por lo tanto, el País no se encuentra preparado para afrontar el gran impacto social y económico que puede generar dicha enfermedad a los pacientes, sus familias y el sistema de salud. En este sentido, se hace urgente y necesario diseñar e implementar estudios de salud pública que permitan caracterizar plenamente a esta población, ya que esta información será de utilidad para la planeación del sistema general de seguridad social en salud, de manera que esté en la capacidad de atender de manera oportuna a esta población, haciendo uso eficiente de los recursos disponibles.

The regulation of APOE transcription in the central nervous system has been partially researched and could be extremely complex. The human APOE exists in three major alleles: ε2, ε3 and ε4, which makes mechanisms of differential expressions of several APOE isoforms a necessity, for a better understanding of its role in the EA, since the variation of locus of alleles results in changes at the APOE level of expression that so far have been related to EA in a controversial manner. The transcription regulation study of these alleles and the variants in their regulator regions are not only helping to explain the EA etiology but to also guide the drug development aimed to EA therapy.
Although the role of MAPK/ERK path in the EA is clear, is also clear that the findings regarding EA molecular patterns can vary because of technical issues in the experimental design of research and the severity of the disease. So the research should aim to a global approach, aiming wide protein networks and networks of signage paths, instead of studying genes in isolation; so one day would be possible to propose therapeutic measures whose effectiveness maintain in the EA process.
The transcriptome studies commonly identify EA altered processes either as specific consequence of the disease or as results of general neurodegeneration effects. It is necessary to carry on more studies that allow to separate the altered processes caused by neurodegenerative process, from the ones that are a direct consequence of EA.
The EA is considered a public health problem since 2012 by the World Health Organization (WHO). To date there is not any update information about the rate and the economic burden generated by this disease in Colombia; therefore the country is not prepared to face the economic and social impact that can produce such disease in patients, their families and the health system. In this regard, it becomes urgent to design and apply Public health studies that allow to fully characterize this population, this information will be useful for the social security general system planning, so that the system is capable to meet in a timely manner, this population, using the available resources efficiently.
