Results
Demographic and clinical data
Demographic and clinical data for all participants are presented in Table 1. The ABD and ABD+ADHD groups did not differ on demographic measures and IQ scores, as estimated with the WASI, which were in the average range. Moreover, there were no significant group differences on clinical measures of manic (YMRS) and depressive (CDRS-R) symptoms.
Neuropsychological assessment results
BRIEF-PR scale results: Table 2 illustrates mean T score and standard deviation for each subscale of the BRIEF-PR in each group. Relative to healthy population norms the two patient groups exhibited significant (p < 0.05) performance deficits in all BRIEF sub-domains, as well as in the BRI, MI and GEC. Post-hoc comparisons indicate that the ABD+ADHD group had significantly higher scores than the ABD group in the Monitoring sub-domain [F (1,27)=8.403; p < 0.007] There was also a non-significant trend such that the ABD+ADHD group had higher scores than the ADB group on the Inhibition sub-scale (p=0.08) (Figure 1).
CPRS-R results: As shown in Table 1, the Conners’ ADHD Index scores were elevated to clinically significant levels in both groups (ABD T=72, in ABD+ADHD T=77); however there were no significant group differences. There was only a non-significant trend (p=0.09) such that ABD+ADHD yielded more elevated scores than ABD on the Inattentive subscale.

Datos demográficos y clínicos
Los datos demográficos y clínicos de todos los participantes se muestran en la tabla 1. No hubo diferencias entre el grupo de los adolescentes con trastorno bipolar (ATB) y el de los adolescentes con trastorno bipolar y trastorno por déficit de atención e hiperactividad (ATB+TDAH) en las variables demográficas, ni en los puntajes de cociente intelectual (CI), cuyos valores estaban dentro del rango promedio según la evaluación de la Escala Abreviada de Inteligencia de Wechsler (WASI). Además, no hubo ninguna diferencia significativa entre los grupos en las medidas clínicas de los síntomas maníacos, evaluados a través de la escala de manía de Young (YMRS), o depresivos, evaluados a través de la escala de clasificación de la depresión infantil revisada (CDRS-R).

Behind them came the soldiers, their guns pointing at the prisoners, their faces like stone. Carlos was at the front, his face twisted in anger, a great red stain on the front of his shirt, spreading as he walked. Rosalba ran forward, but a soldier came up and pushed her to the ground.
When they reached the square, the ragged band of men stopped and a soldier came to the front and began to speak.
“People of the village of Amendillas, we have come to liberate you from the oppressive forces of the Republicans. We are going to Madrid to fight the upstart rebels and restore the old ways.”
The villagers listened, unmoving, their faces entirely without expression. They had been liberated before and had not enjoyed the experience.
The man stood up straighter and went on. “Our orders are to recruit all the able-bodied men for the army and to shoot all those who refuse.” He frowned and gave a piercing look to the band of prisoners. “It would seem that all the men of this village have chosen to oppose us. But we will be merciful on this occasion. We will only shoot the ringleaders and the rest will have a choice.”
He raised his voice. “Bring forth the ringleaders!”
Carlos and two others, the Gálvez brothers, stumbled forward, propelled by a rough thrust from the soldiers behind them.
“No!” screamed Rosalba and tried to get up from the ground. Her head ached abominably and a wave of dizziness swept over her. She sank back onto the pebbles.
There were murmurings from the villagers and shuffling of feet, but nobody attempted to intervene.
As Rosalba struggled once again to get up, she heard Carlos call her name, but before she could stand, the captain barked out the order “Fire!” and she rose in time to see Carlos slump down against the wall, leaving a trail of blood on the plaster. It was the same wall where they had shot her father.

Solo se escucharon murmullos vagos y el crujido de los zapatos sobre la gravilla, pues nadie quiso intervenir.
Mientras Rosalba luchaba por ponerse de pie, una vez más, escuchó a Carlos llamar su nombre, pero antes de que pudiera pararse, el capitán ladró la orden — ¡Fuego! —. 7 INTRODUCTION
7.1 Background
Combined oral contraceptives (COCs) are medications taken orally combining a progestin and an estrogen. The function of the progestin is to inhibit ovulation by a central feedback mechanism resulting in decreased luteinizing hormone (LH) secretion by the pituitary gland. The estrogen component also contributes to contraceptive activity by inhibiting the secretion of follicle-stimulating hormone (FSH) but its major function is to provide stability to the endometrium and consequently to provide acceptable cycle control and bleeding pattern. COCs are widely used in the industrialized world. They have been shown to be highly effective in terms of contraception.
The potent synthetic estrogen, ethinylestradiol (EE), is the most frequently used estrogen in COCs. EE is easily absorbed when administered orally. Products containing EE generally have a good bleeding profile; however EE is also responsible for most of the side effects experienced by COC users. Among those, a rare but potentially very serious event is venous thromboembolism (VTE). The increased risk of thromboembolism among COC users is principally due to the strong impact of EE on hepatic synthesis functions, which leads to an increased production of several proteins involved in the coagulation and inflammatory pathways. As a consequence, an imbalance appears between pro- and anticoagulant circulating factors in favor to pro-coagulation. Beside the thromboembolism risk, EE is also associated with water retention (responsible of breast tension, edema, weight increase, nausea, headache and liability of humor) as well as with an increased risk of cholangiopathy.
To improve the safety profile of COCs, research and development has focused on two main approaches. First, the dose of EE has been progressively lowered from > 75 μg to as low as 10 μg. As EE doses decrease, the incidence of irregular and potentially unacceptable bleeding profiles increases. The second approach has been to replace EE with a naturally occurring estrogen: new COCs containing estradiol (E2) or estradiol valerate (E2V) have recently been developed, with the prospect of decreasing the metabolic impact from the estrogen component of COCs.

7 INTRODUCCIÓN
7.1 Antecedentes
Los anticonceptivos orales combinados (AOC) son medicamentos que se toman por vía oral y se componen de un gestágeno y un estrógeno. La función del gestágeno es impedir la ovulación mediante un mecanismo de retroalimentación central que inhibe la secreción de la hormona luteinizante (LH) secretada por la glándula pituitaria. El estrógeno también contribuye a la función anticonceptiva al inhibir la secreción de la hormona foliculoestimulante (FSH). Sin embargo, su función principal es estabilizar el endometrio y, en consecuencia, brindar cierto control sobre el ciclo menstrual y un patrón de hemorragia vaginal adecuado. Los AOC se usan mucho en los países industrializados y se ha demostrado que son muy eficaces como anticonceptivos.
El estrógeno más utilizado en los AOC es el etinilestradiol (EE), el cual es un estrógeno sintético potente que se absorbe con facilidad al ser administrado por vía oral. Los fármacos que contienen EE generalmente tienen un buen perfil de hemorragia vaginal; sin embargo, el EE es el causante de la mayoría de los efectos secundarios que presentan las usuarias de los AOC, entre ellos la tromboembolia venosa (TEV), un efecto adverso poco frecuente, pero que puede ser muy grave. El riesgo de padecer una tromboembolia entre las usuarias de AOC se debe primordialmente al fuerte impacto del EE sobre las funciones de síntesis hepáticas, lo que da paso a un aumento en la producción de varias proteínas que intervienen en las vías de señalización de los procesos de coagulación e inflamación. Como consecuencia, se presenta un desequilibrio a favor de los factores que favorecen la coagulación en vez de aquellos que la inhiben. Además del riesgo de provocar tromboembolia, a los EE se les asocia con la retención de líquidos, que se manifiesta mediante mastalgia, edema, aumento de peso, náuseas, cefalea y cambios en el estado de ánimo; y a un mayor riesgo de padecer de colangitis.
La investigación y desarrollo se ha centrado principalmente en dos enfoques con el fin de mejorar el perfil de seguridad de los AOC. Primero, se redujo de manera progresiva la dosis de EE de >75 μg a 10 μg. A medida que se disminuye la dosis de EE, aumenta la incidencia de hemorragia vaginal irregular. El segundo enfoque consiste en sustituir el EE con un estrógeno natural. Recientemente, se han desarrollado nuevos AOC que contienen estradiol (E2) o valerato de estradiol (E2V) con la expectativa de disminuir el impacto que genera el estrógeno de los AOC en el metabolismo.
