Portuguese to English: Medication reconciliation General field: Medical
Translation - English Medication reconciliation is the process of reviewing a patient's treatment before and after transitions of care. This tool is key to reducing identified discrepancies and preventing adverse events, as it enables the identification and comparison of all medications used at home prior to hospital admission with those prescribed during hospitalization. Although conceptually simple, its reliable implementation is complex and should therefore be carried out in phases. The aim of this review was to present the development of medication reconciliation — a clinical activity — in promoting patient safety and the rational use of medicines by hospital pharmacists. This process helps minimize medication errors, reduce costs, and improve the overall quality of care. According to the findings, pharmacists are able to obtain a more complete medication history, identifying a higher number of errors. Through their interventions, they can help prevent potential adverse events, reduce patient harm and related costs, and enhance patient safety. Another benefit observed was patient education regarding treatment, which contributes to greater medication adherence and, consequently, improved quality of life. This review reinforces the importance of medication reconciliation as a key tool in preventing medication errors and promoting patient safety.
