Translation - German An arterial hypotonia after the performance of spinal anaesthesia in the course of a caesarean section causes side effects in both the mother and the foetus. Current guidelines recommend a regular and preferably prophylactic use of vasopressors, in particular -agonists like phenylephrine. In addition to a left-lateral relocation of the uterus, an intravenous colloid preload or a crystalloid coload is suggested. The aim is to maintain a systolic arterial blood pressure of at least 90% of the initial base values and to avoid a drop to less than 80% of this baseline. For this purpose, a prophylactic supply with phenylephrine via a syringe pump with an adjustable flow rate is advised. With a dose of 25-50 µg/min, the supply should start immediately after the intrathecal injection of the local anaesthetic and be titrated in accordance with the vital signs. The continuous supply with vasopressors can usually be phased out after the delivery given that the patient is stable. Women with a preeclampsia show a less severe drop in blood pressure. A prophylactic supply with vasopressors is not indicated or, if applicable, the initial flow rate should be reduced. Pregnant women with previous cardiac illnesses must be evaluated individually before surgery.
