Certification of brain death requires demonstration of the cessation of brain stem functions and their irreversibility.
I - Preconditions
In order to establish a diagnosis of brain death, the following conditions must be met:
1) Knowledge of the cause and irreversibility of the clinical situation;
2) Coma with no motor response to painful stimulation in the cranial nerve area;
3) Absence of spontaneous respiration;
4) Confirmation of hemodynamic stability and the absence of hypothermia, endocrine-metabolic changes, central nervous system depressants, and/or neuromuscular blocking agents that could be responsible for the suppression of the functions referred to in the previous paragraphs.
II - Semiology rules
1 - The diagnosis of brain death implies the total absence of the following brainstem reflexes:
a) Photomotor reflexes with fixed pupil diameter;
b) Oculocephalic reflexes;
c) Oculovestibular reflexes;
d) Corneopalpebral reflexes;
e) Pharyngeal reflex.
2 - Performance of the apnea test to confirm the absence of spontaneous breathing.
III - Methodology
Verification of brain death requires:
1) The performance of at least two sets of tests at intervals appropriate to the clinical situation and age;
2) The performance of complementary diagnostic tests, whenever deemed necessary;
3) The performance of brain death tests by two specialist physicians (in neurology, neurosurgery, or with experience in intensive care);
4) None of the physicians performing the tests may belong to teams involved in organ or tissue transplantation, and at least one must not belong to the unit or service where the patient is hospitalized.

O laudo de morte encefálica requer a demonstração da cessação das funções do tronco cerebral e da sua irreversibilidade.
I - Condições prévias
Para o estabelecimento do diagnóstico de morte encefálica é necessário que se verifiquem as seguintes condições:
1) Conhecimento da causa e irreversibilidade da situação clínica;
2) Estado de coma com ausência de resposta motora à estimulação dolorosa na área dos pares cranianos;
3) Ausência de respiração espontânea;
4) Constatação de estabilidade hemodinâmica e da ausência de hipotermia, alterações endócrino-metabólicas, agentes depressores do sistema nervoso central e/ou de agentes bloqueadores neuromusculares, que possam ser responsabilizados pela supressão das funções referidas nos números anteriores.
II - Regras de semiologia
1 - O diagnóstico de morte encefálica implica a ausência na totalidade dos seguintes reflexos do tronco cerebral:
a) Reflexos fotomotores com pupilas de diâmetro fixo;
b) Reflexos oculocefálicos;
c) Reflexos oculovestibulares;
d) Reflexos corneopalpebrais;
e) Reflexo faríngeo.
2 - Realização da prova de apneia confirmativa da ausência de respiração espontânea.
III - Metodologia
A verificação da morte encefálica requer:
1) Realização de, no mínimo, dois conjuntos de provas com intervalo adequado à situação clínica e à idade;
2) Realização de exames complementares de diagnóstico, sempre que for considerado necessário;
3) A execução das provas de morte encefálica por dois médicos especialistas (em neurologia, neurocirurgia ou com experiência de cuidados intensivos);
4) Nenhum dos médicos que executa as provas poderá pertencer a equipes envolvidas no transplante de órgãos ou tecidos e pelo menos um não deverá pertencer à unidade ou serviço em que o doente esteja internado.
