Source text - French La croissance économique est atone, sans espoir d’amélioration à court terme
Le rebond de la croissance observé en 2017 (+1,3%, après +0,6% en 2016) ne s’est pas prolongé en 2018. Le pays est même entré en récession à la fin du premier semestre, du fait de mauvais résultats dans les secteurs agricole et minier. Au troisième trimestre, la croissance a atteint 2,2% grâce aux bonnes performances du secteur manufacturier et celui des services (finances et transport) ainsi qu’à un rebond de la consommation. Sur l’ensemble de l’année 2018, la croissance a atteint 0,8%, bien en deçà des 2% attendus en début d’année. Pour 2019, les prévisions ont été revues à la baisse, aux alentours de 1,2% à 1,3% (Banque centrale, FMI, Banque Mondiale).
L’inflation est maitrisée, elle se situe dans la fourchette de 3%-6% fixée par la Banque centrale : 4,6% en moyenne sur 2018, point bas autour de 4% début 2019 puis accélération pour atteindre 4,5% en mars dernier. Malgré cette inflation relativement stable, la Banque centrale poursuit une politique monétaire prudente (le taux directeur atteint 6,75%). Les taux d’intérêt réels à court terme sont ainsi nettement positifs ce qui soutient le rand dont la volatilité est forte – pays très marqué par la dualité, nombreuses incertitudes qui pèsent sur l’économie et changements constants dans la perception du risque sud-africain par les investisseurs étrangers. Ainsi, depuis 2014, la parité entre le rand et le dollar américain varie dans une fourchette comprise entre 10 et 17 ZAR pour 1 USD. En avril 2019, le taux de change s’établissait à 14 ZAR pour 1 USD, un niveau qui est relativement stable depuis le début de l’année.
Le déficit de la balance des transactions courantes, qui s’était réduit en 2017 (-2,4% du PIB après -2,8% en 2016 – reflet de l’amélioration de la balance commerciale, permise par l’augmentation des prix des matières premières, la réduction des importations de produits agricoles suite à la fin de la sécheresse et l’atonie de la demande intérieure) s’est de nouveau creusé en 2018. Il a atteint -3,5%, principalement en raison de la diminution de l’excédent commercial.
Les flux de capitaux sont essentiels pour combler le déficit chronique de la balance des transactions courantes. Or, la modification de l’équilibre entre Investissements Directs Etrangers (IDE) et investissements de portefeuille, que l’on observe ces dernières années, tend à rendre l’économie sud-africaine vulnérable à une sortie brutale de capitaux. Cela pèse sur l’aptitude du pays à assurer de façon pérenne et à moindre coût l’équilibre de sa balance des paiements. Cela est d’autant plus important, dans un contexte où seule l’agence de notation Moody’s maintient la notation souveraine du pays en catégorie investissement, avec perspective stable – et que la plupart des analystes tablent sur une dégradation de la notation du pays à moyen terme, compte tenu de la situation dégradée de l’économie, des comptes publics et de l’entreprise publique Eskom qui pèse sur ses derniers.
Translation - English Economic growth is listless, with no hope of short-term improvement
The growth rebound observed in 2017 (+ 1.3%, after + 0.6% in 2016) did not extend into 2018. The country even entered into recession by the end of the first half of the year, as a result of poor results in the agricultural and mining sectors. In the third quarter, growth reached 2.2% as a result of the good performance of the manufacturing and service sectors (finance and transport) as well as a rebound in consumer spending. In 2018 as a whole, growth reached 0.8%, well below the 2% expected at the beginning of the year. For 2019, forecasts were revised downwards in the region of 1.2% to 1.3% (Central Bank, IMF, World Bank).
Inflation is under control, within the range of 3% -6% set by the Central Bank: 4.6% on average in 2018, at its lowest point around 4% at the start of 2019 then inflation accelerated to reach 4.5% last March. Despite this relatively stable inflation, the Central Bank is pursuing a prudent monetary policy (the key interest rate is 6.75%). Real short-term interest rates are thus clearly positive, which support the highly volatile Rand, - in a country marked by duality, where numerous uncertainties weigh on the economy and there are constant changes in the perception of South African risk by foreign investors. Thus, since 2014, the parity between the Rand and the US dollar has varied within a range of 10 to 17 ZAR for 1 USD. In April 2019, the exchange rate was fixed at ZAR 14 to USD 1, a level that has been relatively stable since the start of the year.
The current account deficit, which had narrowed in 2017 (-2.4% of GDP from -2.8% in 2016) - a reflection of the improvement in the trade balance, made possible by the increase in commodity prices, the reduction in imports of agricultural products following the end of the drought and the sluggishness of domestic demand) widened again in 2018. It reached -3.5%, mainly due to the decrease in trade surplus.
Capital inflow is essential to narrow the chronic deficit in the current account. However, the change in the balance between Foreign Direct Investments (FDI) and portfolio investments, seen over the past few years, tends to make the South African economy vulnerable to a sudden outflow of capital. This affects the country's ability to ensure a sustainable and balanced lower cost in its balance of payments. This is even more important, in a context where only the credit rating agency Moody's maintains the country's sovereign rating in investment grade, with a stable outlook - and most analysts forecast a downgrade of the country's rating in the medium term considering the deteriorating economic situation, public accounts and the state-owned company Eskom which is an added burden.

Source text - French Les principales fièvres récurrentes de nature infectieuse sont les infections bactériennes focales : fièvres canalaires (infections des voies biliaires, sigmoïdite, infections urinaires, etc.) ou en rapport avec un foyer bactérien profond localisé (dentaire, ORL, rétropéritonéal, osseux, etc.). De nos jours, du fait de la pratique rapide de la tomodensitométrie thoraco-abdomino-pelvienne dans l’exploration des fièvres prolongées, les infections bactériennes focales posent moins de problème diagnostique. Il faut toutefois souligner que certaines prostatites ou mastoïdites peuvent rester de diagnostic difficile. L’existence de matériel étranger (matériel endovasculaire, matériel d’ostéosynthèse, etc.) doit faire évoquer l’hypothèse d’une infection persistante locale. Le corps étranger peut être méconnu du patient et il faut savoir rechercher un antécédent chirurgical même très ancien. Une thrombose septique des veines jugulaires ou sous-clavières mérite d’être recherchée chez un patient présentant une fièvre récurrente et ayant récemment eu, lors d’une hospitalisation, un cathéter veineux central.
Les endocardites infectieuses représentent une cause de fièvre intermittente à ne pas méconnaître. Toute fièvre intermittente chez un patient atteint de valvulopathie ou de stimulateur cardiaque justifie la pratique d’hémocultures et d’une échographie cardiaque. Il faut rappeler que les endocardites sur stimulateur cardiaque peuvent se manifester par des emboles pulmonaires septiques à répétition, responsables d’un tableau de fièvre récurrente avec symptômes respiratoires.
Translation - English The main recurring infectious fevers are focal bacterial infections: ductal fevers (infections of the biliary duct, diverticulitis, urinary infections etc.,) or are related to localized deep bacterial focus (dental, ENT, retroperitoneal, bone tissue, etc.). These days, as a result of the rapid practice of thoraco-abdomino-pelvic computed tomography in the exploration of prolonged fever, focal bacterial infections pose less of a diagnostic problem. However, it should be stressed that certain types of prostatitis or mastoiditis may remain difficult to diagnose. The existence of foreign material (endovascular material, osteosynthetic material etc.,) could suggest a hypothetical persistent local infection. The foreign body may be unrecognized by the patient and it would be necessary to know how to check for surgical history – no matter how old. Septic thrombosis of the jugular or sub-clavicular veins should be investigated in patients presenting with recurring fever and who may recently have had a central venous catheter while in hospital.
Infectious endocarditis presenting causes of intermittent fever should not be ignored. Any fever in patients presenting with valve disease or on a pacemaker justifies an analysis of blood cultures and cardiac sonography. It should be noted that endocarditis patients with pacemakers may manifest with repeated pulmonary embolisms, which presents a picture of recurrent fever with respiratory symptoms.
