La cholangite biliaire primitive (CBP) est une maladie chronique du foie qui touche principalement les femmes de plus de 40 ans.
Elle se caractérise par une inflammation progressive des petits canaux biliaires situés à l'intérieur du foie.
Lorsque ces canaux sont endommagés, la bile s'accumule dans le foie et peut, à terme, provoquer une fibrose, puis une cirrhose.
Les symptômes les plus fréquents sont la fatigue et des démangeaisons, parfois intenses, qui peuvent perturber le sommeil.
Cependant, de nombreux patients ne présentent aucun symptôme au moment du diagnostic.
Le diagnostic repose généralement sur une prise de sang montrant une élévation des phosphatases alcalines et la présence d'anticorps antimitochondries.
Le traitement de première ligne est l'acide ursodésoxycholique, qui se prend par voie orale tous les jours.
Chez la plupart des patients, ce traitement ralentit l'évolution de la maladie.
Lorsque la réponse est insuffisante, votre hépatologue peut vous proposer un traitement de deuxième ligne.
Un suivi régulier est indispensable : bilans sanguins, mesure de l'élasticité du foie et, si nécessaire, densitométrie osseuse.
Ne modifiez jamais votre traitement sans avis médical, et signalez à votre médecin tout nouveau symptôme, en particulier une jaunisse ou un gonflement du ventre.
Translation - English Primary Biliary Cholangitis: Information for Patients
Primary biliary cholangitis (PBC) is a chronic liver disease that primarily affects women over the age of 40.
It is characterized by progressive inflammation of the small bile ducts within the liver.
When these ducts are damaged, bile builds up in the liver and can eventually lead to fibrosis, followed by cirrhosis.
The most common symptoms are fatigue and itching, which can sometimes be severe and disrupt sleep.
However, many patients have no symptoms at the time of diagnosis.
Diagnosis is generally based on a blood test showing elevated alkaline phosphatase levels and the presence of antimitochondrial antibodies.
The first-line treatment is ursodeoxycholic acid, taken by mouth daily.
In most patients, this treatment slows the progression of the disease.
If your response to treatment is inadequate, your hepatologist may recommend a second-line treatment.
Regular monitoring is essential and includes blood tests, liver stiffness measurements and, if necessary, a bone density scan.
Never change your treatment without medical advice, and inform your doctor of any new symptoms, particularly jaundice or abdominal swelling.
