Source text - English The vascular endothelium of tumour tissue, which differs in several ways from that of normal tissues, is a potential target for selective anticancer therapy. By contrast with antiangiogenic agents, antivascular agents target the endothelial cells of existing tumour blood vessels, causing distortion or damage and consequently decreasing tumour blood flow. DMXAA (5,6-dimethylxanthenone-4-acetic acid), a low-molecular-weight drug, has a striking antivascular and in some cases curative effect in experimental tumours. Its action on vascular endothelial cells seems to involve a cascade of events leading to induction of tumour haemorrhagic necrosis. These events include both direct and indirect effects, the latter involving the release of further vasoactive agents, such as serotonin, tumour necrosis factor, other cytokines, and nitric oxide from host cells. Phase I clinical trials of DMXAA have been completed and the next challenge to face is how the antivascular effect of this drug should be exploited for the treatment of human cancer.

Contrairement aux agents anti-angiogéniques, les agents anti-vasculaires ciblent les cellules endothéliales des vaisseaux sanguins tumoraux déjà existants, causant des déformations et des dommages et diminuant en conséquence le débit sanguin de la tumeur. Le DMXAA (acide 5,6-dimethylxanthenone-4-acétique), une substance à faible poids moléculaire, a un effet anti-vasculaire impressionnant sur les tumeurs expérimentales et parfois même un effet curatif. Son action sur les cellules endothéliales des vaisseaux sanguins semble induire, en cascade, une nécrose hémorragique de la tumeur. Cette action repose à la fois sur des effets directs et indirects : parmi ces derniers la libération d'agents vasoactifs supplémentaires, comme la sérotonine, le facteur de nécrose tumorale, d'autres cytokines et de l'oxyde nitrique venant des cellules hôtes. La Phase I des essais cliniques du DMXAA est terminée et le prochain défi sera de voir comment utiliser l'effet anti-vasculaire de cette substance dans le traitement du cancer chez l'homme.
