The staff of the Penn State/Good Samaritan Family Practice Center are conducting a research survey in order to help us serve you better. We are trying to better understand the reasons why some patients miss appointments. The following questions will help us do just that. Your participation is entirely voluntary, and your responses will be completely confidential. Your name and other identifying information will not be associated with the questionnaire. It should take about 20 minutes to complete. We anticipate that about 100 people will participate in this study, and will be conducting it between July and September, 2003. Your filling out this survey implies your consent to participate in this study.
