Source text - English To understand how leads work, it is necessary to step back on basic fundamentals : potential differences and vectors.
ECG is the recording of electrical potential differences going through the heart. The propagation of electrical potentials inside the heart represents in fact the propagation of a depolarized zone (positive) inside a repolarized myocard (negative). Therefore, it is the importance, the speed and the average direction of this electrical signal that are recorded by the ECG machine. To say it another way, ECG is the recording of the vector representing the potential difference between two points (positive-negative).
As a result, a repolarized (all negative) or fully depolarized (all positive) resting heart will give a “null” or a flat recording : the corresponding printout will be the baseline or isoelectric line.
To record this potential difference it is necessary to have 3 electrodes : one positive electrode, one negative electrode and one neutral electrode. The position of the neutral electrode is indifferent and can be placed anywhere on the body.
The positive electrode will record activity and assigned it a positive value if the electrical activity moves toward the positive electrode. On the opposite, when the flow of electrical activity is moving away from the positive elctrode, the ECG machine will assign it a negative value.

EKG er registreringen af de elektriske spændingsforskelle der passerer gennem hjertet. Den elektriske spændingsforskel der forplanter sig i hjertet, repræsenterer faktisk forplantningen af en depolariseret (positiv) zone inde i et repolariseret (negativt) myokardie. Det er derfor det elektriske signals vigtighed, hastighed og gennemsnitlige retning der bliver registreret af EKG-apparatet. Som følge heraf vil et repolariseret (helt negativt) eller et depolariseret (helt positivt) hjerte i hvile give en 0-linje, d.v.s. en lige linje: det print man får, vil vise basislinjen eller den isoelektriske linje.
For at registrere den potentielle forskel er det nødvendigt at bruge 3 elektroder: en positiv elektrode, en negativ elektrode og en neutral elektrode. Placeringen af den neutrale elektrode er underordnet; den kan placeres hvor som helst på kroppen.
Den positive elektrode vil registrere aktiviteten og angive en positiv værdi hvis den elektriske aktivitet bevæger sig mod den positive elektrode. Når den elektriske strøm bevæger sig bort fra den positive elektrode, vil EKG-apparatet tildele den en negativ værdi.
